PERİMENOPOZAL DÖNEMDE KENDİNİ VAJEN METASTAZI İLE GÖSTEREN GESTASYONEL TROFABLASTİK HASTALIĞIN YÖNETİMİ: OLGU SUNUMU

Abstract

Amaç: Mİnvazif mol; molar gebelikler olarak kabul edilen parsiyel veya tam hidatidiform molun, uterusu invaze etmesi veya vücudun diğer kısımlarına yayılım göstermesiyle oluşan bir durumdur. İnvazif mol tanısı; hidatidiform molun myometriuma invazyonu veya vajen gibi bölgelere yayılan metastatik lezyonlarda villus yapısının gösterilmesi ile konur. Molar gebelik boşaltılması sonrasında görülen nadir invazif mol vakasını sunduk. Olgu Sunumu: Hasta 49 yaşındaydı, 8 haftalık amenore sonrası, beta hcg değeri 131014 mIU/mL ve minimal vajinal kanama ile başvurdu. Yapılan transvajinal ultrasonografide genişlemiş endometrial kavitede değişik büyüklüklerde anekoik kistler saptandı. Küretaj sonucu patolojisi mol hidaditiform geldi. Hasta molar boşaltımı takiben vajen sağ duvarında 1-2 cm çapında metastatik lezyonla geldi. Hastada vajinal metastatik lezyonlu invazif mol tanısı koyduk. Hasta düşük riskli metastatik gestasyonel trofoblastik hastalık olarak kabul edildi, WHO skoru 6 olarak bulundu. Hasta haftalık metotreksat tedavisi aldı, 8 hafta sonra beta hcg değeri normal sınırlara geldi. Tartışma: Vajen, trofoblastik hastalıkların akciğerlerden sonra ikinci en sık metastaz yeridir. Kolay kanayabilen ve damarlanması artmış metastatik lezyonlar ciddi kanamalara neden olabilir. Vajinal metastazın asıl tedavisi hala kemoterapidir.

THE MANAGEMENT OF VAGİNAL METASTASİS OF THE GESTATİNAL TROPHOBLASTİC DİSEASE İN PERİMENOPAUSAL WOMAN: CASE REPORT

Abstract

Objective: Invasive mole is a condition where a molar pregnancy, such as a partial hydatidiform mole or complete hydatidiform mole, invades the wall of the uterus, metastasizing to other parts of the body. The diagnosis of invasive mole rests on the demonstration of complete hydatidiform mole invading the myometrium or the presence of villi in the metastatic lesion as like vagina. We present a rare case of an invasive mole, which developed following the evacuation of a molar pregnancy. Case Report: She presented 49-year-old with eight weeks amenorrhea, a β-HCG value of 131014 mIU/mL and minor vaginal bleeding. The ultrasonographic examination showed a distended endometrial cavity containing innumerable, variably sized anechoic cysts. A curettage was performed and the pathology revealed a complete hydatidiform mole. The patient presented with a vaginal metastatic lesion on the right wall about 1-2 cm after molar evacuation. We diagnosed invasive mole with vaginal metastatic lesion. She was considered as having low risk metastatic GTN, as her modified WHO score was 6. Patient received weekly MTX and was followed up for 8 weeks after beta hCG levels were within normal limits. Discussion: The vagina, next to the lung, is the second most common metastatic site in trophoblastic tumors. The friable and hypervascular nature of a metastatic lesion places the patient at risk for significant hemorrhage. The main treatment for vaginal metastasis is still chemotherapy.
