Sıvı bazlı teknik ile yapılan servikal kanser tarama testinde test yetersizliği açısından sürüntü alma sırasında smear fırçası döngü sayısının önemi

Abstract

Amaç: Sıvı bazlı servikal smear tarama testinde, servikal hücre örneği, smear fırçasının ucu servikal kanala yerleştirilip fırçanın döndürülmesi suretiyle elde edilmektedir. Bu çalışmanın amacı, smear fırçasının döngü sayısı ile smear yeterliliği ilişkisinin araştırılmasıdır. Materyal-metod: Jinekoloji polikliniğine başvuran smear tarama testi planlanan reproduktif dönemdeki kadınlar bu çalışma için seçildi. Çalışmaya dahil edilme kriterlerini karşılayan hastalar jinekolojik muayene masasına alındı. Smear fırçasının ucu servikal os ile temas ettirildi ve fırça döndürülerek smear örneği alındı. Smear fırçasının dönme hareketine göre 1 tur (3600), 2 tur ve 3 tur olmak üzere üç farklı grup oluşturuldu. Gruplar smear örneğinde hücre yeterliliği açısından kruskal-wallis testi ile istatistiksel olarak karşılaştırıldı. Bulgular: Ortalama yaşı 35.5 ± 6.1 yıl (min: 21 maks: 50) olan toplam 451 hasta değerlendirildi. Bir tur (3600), 2 tur ve 3 tur rotasyon grubundaki olgu sayıları sırayla 150, 151 ve 150 idi. 451 olgunun 8 inde smear yetersizliği (% 1.78) saptandı. Bir tur grubunda, iki tur grubunda ve üç tur grubunda smear yetersizliği sayısı sırasıyla 1, 3 ve 4 olarak bulundu (P=0.176). ASCUS (Atypical squamous cells of unknown significance) bir tur grubunda 2, iki tur grubunda 4, üç tur grubunda 2 hastada saptandı (P= 0.751). Yüksek derece preinvaziv lezyon saptanmadı. Sonuç: Çalışmanın sonuçlarına göre, sıvı bazlı teknikte yetersiz smear oranları düşüktür. Smear fırçasının bir tur döndürülmesi ile elde edilen smear örneklerindeki hücre yetersizliği oranları, fırçanın 2 veya 3 tur döndürülmesiyle elde edilen smearler ile benzerdir. Bu sonuç, sıvı bazlı teknik ile yapılan smear tarama testinde, süpürge tarzı smear fırçası ile 1 tur döndürülerek alınan smear örneğinin değerlendirme için yeterli olduğunu göstermektedir.

The importance of the number of smear brush cycles during swab collection in terms of test failure in cervical cancer screening test performed with liquid-based technique

Abstract

Aim: The aim of this study is to investigate the relationship between cell adequacy of smear sample and rotation movement of smear brush in cervicovaginal smear procedure. Material - methods: Women in reproductive period, who were scheduled for smear screening test, were selected for the study. The patients who met the inclusion criteria were taken to the gynecological examination table. The tip of the smear brush was contacted with the cervical os and the smear sample was obtained by rotating the brush. Three different groups were formed to be 1 turn (3600), 2 turns and 3 turns according to the rotational movement of the smear brush. The groups were compared statistically with the kruskal-wallis test in terms of cell adequacy in the smear sample. Results: A total of 451 patients with a mean age of 35.5 ± 6.1 years (min: 21 max: 50) were evaluated. The number of smears for one round (3600), 2 rounds, and 3 rounds were 150, 151 and 150, respectively. Smear cell insufficiency was detected in 8 patients (1.78%). Insufficient smear were found as 1, 3 and 4 in the one-round group, two-round group and three-round group, respectively (P = 0.176). ASCUS (Atypical squamous cells of unknown significance) was detected in 2 patients in one round group, 4 in the two-round group, and 2 in the three-round group (P = 0.751). High-grade preinvasive lesion was not detected. Conclusion: According to the results of the study, insufficient smear rates are low in the liquid based technique. The cell deficiency rates in the smear samples obtained by rotating the smear brush by one turn are similar to the smears obtained by rotating the brush 2 or 3 turns. This result shows that the smear sample taken by turning 1 turn with a broom-style smear brush in the smear scanning test performed with the liquid-based technique is sufficient for evaluation.
