Abstract
N.A., 39 yaşında, G:1 P:0 hasta, 20 haftalık gebelik ve geçirilmiş sol ooferektomi ile hastanemize başvurdu. ICSI ile elde edilmiş olan gebeliği dış merkezde takip edilen hastanın 18. gebelik haftasında rutin antenatal takipleri esnasında ultrasonografide 17x14x6 cm'lik solid- kistik alanlar içeren kitle tespit edilmesi üzerine exploratif laparotomi uygulanarak, sol ooferektomi yapılmış ve patoloji sonucu expansif tipte invazyon gösteren müsinöz karsinom olarak rapor edilmiştir. İntraoperatif explorasyonda, herhangi bir asit, lenfadenopati ya da metastaz bulgusuna rastlanmadığı belirtilmiştir. 20. haftadan itibaren takiplerine merkezimizde devam edilen hastaya, 4 kür taxol + karboplatin kemoterapisi uygulanarak 35. haftada gebeliği sezaryen ile sonlandı rıldıktan sonra aynı seansta total abdominal histerektomi + unilateral (sağ) salpingooferektomi + pelvik ve paraaortik lenf nodu disseksiyonu + omentektomi + apendektomi + tanı amaçlı peritonel lavajı uygulanmıştır.Postoperatif patolojik inceleme sonucunda alınan tüm organ-doku ve örneklerde tümör negatif olarak tespit edilmiştir. Hastamız, postoperatif 3. günde şifa ile taburcu edilmiştir. Postoperatif 5. aya kadar takiplerinde hastalık tespit edilmemiş olup, bebeği ise sağlıklı bir büyüme göstermektedir.

Abstract
A 39- year old woman (G: 1 P: 0) in her first pregnancy which was
achieved by ICSI was referred to our hospital with the diagnosis of
operated left mucinous ovarian carsinom and pregnancy in 20 weeks’
gestation. Her antenatal follow up was performed in a different center
with the incidental detection of a cystic- solid mass measuring 17*14*6
cm in 18. weeks’ gestation. An exploratory laparotomy was performed
and there was no ascites, lymphadenopathy or evidence of metastasis.
Unilateral oopherectomy was performed and an expansive
ovarian musinous carsinom was reported after pathological examination.
After 20. weeks of gestation, her antenatal follow up was held in
our hospital. 4 courses of paclitaxel + carboplatin chemotherapy was
given to the patient and in 35. weeks of pregnancy, after cesarean section,
we performed total abdominal hysterectomy + unilateral (right)
salpingoopherectomy + pelvic and paraaortic lymphadenectomy +
omentectomy and appendectomy. The pathological examination revealed
negative peritoneal washings and lymph nodes. In postoperative
3rd day she was externed with no complications. She has now had
a follow up of 5 months and is disease free. Her baby is showing normal
milestones and growth.
