A Rare Case of Deep Vein Thrombosis: Paget-Schroetter Syndrome

Abstract

Paget-Schroetter syndrome is a rare form of upper extremity deep vein thrombosis (DVT), accounting for approximately 0.5%–1% of all DVT cases, and typically caused by repetitive upper extremity activities. It is also referred to as the axillary-subclavian vein effort thrombosis and generally results from chronic compression of the subclavian vein at the thoracic outlet level. We report the case of a 23-year-old man who developed pain and swelling in the right shoulder and upper extremity following weightlifting. No risk factors such as trauma, smoking, venous catheterization, malignancy, hematologic disorder, or chronic disease were present. Venous Doppler ultrasonography revealed an acute thrombus in the subclavian vein, whereas computed tomographic angiography showed no arterial involvement. The patient’s symptoms improved following early anticoagulant therapy.

Nadir Bir Derin Ven Trombozu: Paget-Schroetter Sendromu

Öz

Paget-Schroetter sendromu, aşırı ve tekrarlayan üst ekstremite aktivitelerinin neden olduğu ve tüm derin ven trombozları içinde %0,5-1 arası görülebilen nadir bir derin ven trombozudur. Bu nedenle aksiller-subklaviyan ven efor trombozu olarak da isimlendirilebilir. Bu durum genellikle, subklaviyan venin torasik çıkış seviyesinde kronik kompresyonu sonucu meydana gelir. Olgu sunumumuzda 23 yaşında erkek hastada, ağırlık kaldırma sonrası sağ omuz bölgesi ve sağ üst ekstremitede ağrı, şişlik yakınması olan olgu sunulmuştur. Olguda mevcut olan derin ven trombozunun etyolojisinde travma, sigara kullanımı, geçirilmiş venöz kateter girişimi, malignite, hematolojik bozukluk veya kronik hastalık öyküsü gibi herhangi bir risk faktörü bulunmuyordu. Hastanın çekilen bilgisayarlı tomografik anjiosunda arteryel yapılarda trombozu bulunmuyordu. Yapılan venöz doppler ultrasonografisinde subklaviyan vende akut trombüs saptandı. Hastada erken dönemde yapılan antikoagülan tedavi ile mevcut semptomlarında iyileşme sağlandı.
