Late Migration of a Polymer Clip After Sleeve Left Upper Lobectomy: A Rare Delayed Complication

Abstract

Polymer vascular clips are commonly used in thoracic surgery to ensure secure ligation of pulmonary vessels and are generally considered safe. Clip migration is well described in various abdominal procedures but is extremely uncommon after pulmonary resections. We report the case of a 35-year-old man who underwent sleeve left upper lobectomy for a malignant epithelial tumor and remained disease-free for more than four years. The patient unexpectedly expectorated a foreign body following a mild coughing episode, which was identified as the polymer clip originally used to ligate the lingular artery. Computed tomography revealed no fistula, tract formation, or erosion, and bronchoscopy demonstrated completely normal airway anatomy without inflammatory or erosive changes. This case represents an unusual example of very late, entirely asymptomatic migration of a polymer vascular clip without any radiologic or endoscopic abnormality. Clip migration should be considered in the differential diagnosis of unexpected foreign-body expectoration, even years after thoracic surgery.

Sleeve Sol Üst Lobektomi Sonrası Polimer Klipsin Geç Dönem Migrasyonu: Nadir Bir Gecikmiş Komplikasyon

Abstract

Polimer vasküler klipsler, pulmoner damarların güvenli ligasyonunu sağlamak amacıyla torasik cerrahide yaygın olarak kullanılmakta olup genel olarak güvenli kabul edilmektedir. Klips migrasyonu, çeşitli abdominal cerrahi girişimlerde iyi tanımlanmış bir komplikasyon olmakla birlikte, pulmoner rezeksiyonlar sonrasında son derece nadirdir. Bu yazıda, malign epitelyal tümör nedeniyle sleeve sol üst lobektomi uygulanan ve dört yılı aşkın süredir hastalıksız izlenen 35 yaşında bir erkek hastanın olgusu sunulmaktadır. Hasta, hafif bir öksürük atağı sonrasında beklenmedik şekilde yabancı bir cismi ağız yoluyla çıkarmış ve bu cismin, operasyon sırasında lingular arterin ligasyonu için kullanılan polimer klips olduğu saptanmıştır. Bilgisayarlı tomografide fistül, trakt veya erozyon bulgusuna rastlanmazken, bronkoskopide inflamatuvar ya da eroziv değişiklik olmaksızın tamamen normal hava yolu anatomisi izlenmiştir. Bu olgu, herhangi bir radyolojik veya endoskopik anormallik olmaksızın gelişen, son derece geç dönemde ortaya çıkan ve tamamen asemptomatik seyreden polimer vasküler klips migrasyonunun nadir bir örneğini temsil etmektedir. Torasik cerrahiden yıllar sonra dahi gelişen beklenmedik yabancı cisim ekspektorasyonlarında klips migrasyonu ayırıcı tanıda mutlaka akılda tutulmalıdır.
