We Evaluated Hundred Percent of Type 2 Diabetes Cases Which Were Registered in Diabetes Polyclinics; The Insulin is Our Rescuer

Abstract

Aim: Diabetes need disciplined treatment and follow-up from time-of diagnosis to death. Achieving glisemic control and A1C targets could be considered the greatest steps forward to prevent complications. We observed hundred type 2 diabetes patients at our policlinic..Material and Methods: Type 2 diabetes patients who applied our polyclinic recently and the ones who have been followed for five years are included cross-sectional study. Patients’ metabolic status and complications are evaluated at “previous” and “control” visits randomly at least last three months.Results: 59 male/41 female type 2 diabetes were included in the study. The mean age was 58, duration of diabetes was 10,5 years, BMİ was 29 kg/m2. The mean pre- and post-evaluation period was 9,5 months. İn the pre-admission period prePBG was 150 mg/dL, postPBG was 226 mg/dL, A1C 7,7 and 44% of patients with A1C≤7. İn the post-admission prePBG was 139 mg/dL, postPBG was 207 mg/dL, A1C 7,3 and 53% of patients with A1C≤7. Having compared these data, it was revealed that all of them were statistically significant. Lipid profiles were 196 mg/dL for total cholesterol, 182 mg/dl for triglyceride, 45 mg/dL for hdl-cholesterol and 118 mg/dl for ldlcholesterol. Diabetic nephropathy was found in 37, neuropathy in 28, retinopathy in 23, coronary artery disease in 34 and hypertension in 68 person. With regard to treatment, 90% of the cases were taking metformin, 10% SU, 12% glinide, 36% DPP4 inhibitor, 4% glitazone, 1% GLP1 analgesics and 59% insulin treatment. 16% of the insulin treatments received oral anti-diabetic drug + basal insulin, 43% received intensive insulin therapy. Treatment changes were made in 57% of patients. Treatment changes were initiated with 15 DPP4 inhibitors, 2 sulfonylurea, 4 metformin, 5 GLP1 analgesics, 27 insulin treatments. Conclusion: It was concluded that our diabetes polyclinic is in a good position to reach glucose A1C targets in individuals with type 2 diabetes; especially early combination and timely insulinisation play an important role in reaching the target.
