A Case With Type 2 Diabetes Mellitus Complicated With Diabetic Hand and Literature Review

Abstract

Development of “diabetic hand syndrome” is complex and multifactorial. Its risk factors and pathogenesis are similar to diabetic foot. We report here a case in which diabetic hand syndrome arises from vascular rare complications, especially.A 54 year-old female patient was diagnosed with type 2 diabetes 20 years ago and has been using insulin for the last 10 years. Lately, a left fistula has been opened. Complaints about the coldness of the left hand started two months ago. The fistula is closed because of the leak. About a month ago, amputation was made due to necrosis of the left hand third finger. In the following period, blackness on the third and fifth fingers of the left hand led to height in blood glucose. Intensive insulin therapy and dose titration were performed to control blood glucose levels. The medical treatment he was receiving was directed to the operation with appropriate parenteral antibiotherapy, analgesia and maximum medications under appropriate hemodialysis conditions.As seen in our case, maximum treatment should be done with multidisciplinary approach to diabetic foot and “diabetic hand syndrome” cases. Diyabetik El Komplikasyonu Gelişen Mikrovasküler ve Makrovasküler Komplikasyonların Birlikte Olduğu Bir Tip 2 Diyabet Olgusu

Abstract

GirişDiyabetik el sendromu gelişimi kompleks ve multifaktöriyeldir. Risk faktörleri ve patogenezi diyabetik ayak ile benzerlik göstermektedir. Burada diabetes mellitus tanısı aldıktan yıllar sonra vasküler komplikasyonların oluştuğu, özellikle nadir komplikasyonlardan diyabetik el sendromunun ortaya çıktığı bir olguyu sunmaktayız.OlguEllidört yaşında kadın hasta 20 yıldır tip 2 diyabet tanısıyla son 10 yıldır insulin kullanmaktadır. Evde kan şekerleri 300mg/dl üzerinde seyretmekteymiş. Sol koldan fistül açılmış ve iki ay önce sol elde soğukluk morarma şikâyetleri başlamış. Kaçak nedeniyle fistül kapatılmış. Yaklaşık bir ay önce sol el üçüncü parmağa nekroz nedeniyle ampütasyon yapılmış. Takip eden dönemde sol el üçüncü ve beşinci parmaklarda siyahlaşma ile kan şekerlerinde yükseklik nedeniyle yönlendirilmiş. Yoğun insülin tedavisi ve doz titrasyonu yapılarak kan şekeri kontrolü sağlandı. Almakta olduğu medikal tedavi, uygun hemodiyaliz şartlarında paranteral antibioterapi, analjezi ve maksimum medikasyonla operasyona yönlendirildi.TartışmaOlgumuzda görüldüğü gibi diyabetik ayak gibi “diyabetik el sendromu” da vasküler bir komplikasyon olarak görülebilir. “Diyabetik el” tespit edilen olgulara multidisipliner maksimum tedavi yapılmalıdır. Tromboembolilere yönelik primer koruyucu ve sekonder korunma yöntemlerini uygulamayı gerektirmektedi
