Relationship Between Metabolic Parameters and Helicobacter Pylori Infections With Upper Gastrointestinal System Endoscopy and Gastric Histopathology Results in Patients With Dyspeptic Type 2 Diabetes Mellitus

Abstract

Aim: In this study, we examined the dyspepsia symptoms in diabetics, the organic and functional dyspepsia of the subtypes of symptoms and their distinctive values, the relationship between Helicobacter pylori (H. pylori, HP) infection and specific dyspepsia types, symptoms and diagnoses, metabolic and clinical factors.Material and Methods: 44 type 2 diabetic patients and 69 non-diabetic patients were included in the study. Histological examination results and HP density results of two endoscopic (gastric antrum and corpus) biopsy materials were determined in each case, compared with control group. The relationship between HP and clinical and metabolic characteristics was assessed in diabetics. Results: The mean age of the diabetic patients was 55.29 (± 8.22) and the ratio of male patients was 20.45% (n: 9). Functional dyspepsia was found in 61.4%, duodenal ulcer in 6.8%, gastric ulcer in 6.8%, reflux esophagitis in 6.8%. In diabetic cases, HP positivity rates in dyspepsia subgroups were similar. In our diabetic cases, the incidence of HP infection was low in non-diabetic patients and the difference was statistically significant (p = 0.047). The level of inflammation in diabetic patients was slightly higher but not significant (p <0.10). The activation level was significantly higher in diabetics (p <0.02). Peripheral neuropathy was detected in 77.3% (n: 34) of diabetic patients. In these cases, the HP frequency was 61.8%. Peripheral neuropathy was present in 84.0% of HP positive cases. Conclusion: In our study, in Type 2 diabetic cases, when some symptoms were found to be associated with organic dyspepsia and peptic ulcer disease, they are not enough specific and based on the symptoms the differential diagnosis of dyspepsia can not be made. Endoscopy and other diagnostic methods are required for differential diagnosis.HP infection in diabetes mellitus can be detected at low rates.Diabetes mellitus has been found to have an increasing effect on activation in gastric physiopathological changes, with a weak interaction between the clinical and metabolic conditions and the bacteria
