Abstract

Genellikle tümör nedeniyle uygulanan sakrektomi operasyonları sonrasında geniş yumuşak doku defektleri de oluşmaktadır. Bu hastalarda geniş cerrahi rezeksiyon ve uygulanmış radyoterapi gibi nedenlerle onarımda çevre dokuların kullanımı sınırlı hale gelebilir. Bu nedenle uzak fleplerin kullanımı gündeme gelebilir. Fakat bu kez de alıcı damarların olası hasarı serbest flep cerrahisini engelleyebilir. Bu gibi durumlarda transabdominal vertikal rektus abdominis kas-deri flebi, onarımda uygun seçenek olabilir. Bu çalışmada kordoma nedeniyle sakrektomi uygulanan ve oluşan defektin onarımı için transabdominal vertikal rektus abdominis kas-deri flebi kullanılan bir olgu sunulmuştur. Flebin uygulanışı ve bu bölgenin onarımı için tanımlanmış diğer tedavi seçenekleri gözden geçirilmiştir.

THE USE OF TRANSABDOMINAL VERTICAL RECTUS ABDOMINIS FLAP IN RECONSTRUCTION OF SACRECTOMY DEFECTS: A CASE REPORT

Abstract

Large soft tissue defects may accompany to sacrectomy operations which are frequently performed for tumor ablations. As some factors like extended surgery or applied radiotherapy may limit the usage of local tissues in those patients, then, free flaps may be required. However, possible recipient vessel injuries may restrict free flap surgery. In these circumstances, transabdominal rectus abdominis musculocutaneous flap may be the appropriate choice of reconstruction. In this study, a case with sacrectomy defect due to cordoma resection who has been reconstructed by a transabdominal vertical rectus abdominis musculocutaneous flap is presented. The application of the flap and other reconstructive techniques described for the reconstruction of the sacral area are reviewed.
