Persistan Medyan Arter ve Bifid Medyan Sinir: Olgu Sunumu

Abstract

Bifid medyan sinir ve persistan medyan arter anomalisi genellikle asemptomatik seyreden ve çoğunlukla intraoperatif tanı konulan bir anomalidir. Persistan medyan arter embriyolojik dönemde mevcut olan el ve ön kolu besleyen medyan arter kalıntısıdır. Bifid medyan sinir genellikle rastlanan bir durumdur. Bifid medyan sinir ve medyan arter birlikteliği nadir saptanan bir durumdur. Bu makalede asemptomatik olarak mevcut olan, intraoperatif tanı almış bifid medyan sinir ve medyan arter birlikteliği sunulmuştur. Bifid medyan sinir ve medyan arter birlikteliği popülasyonda %1-2 oranında görülen bir anomalidir. Bu anomali asemptomatik olduğu gibi bazı durumlarda akut veya kronik semptomlar şeklinde bulgu vermektedir. Akut semptom olan vakalarda medyan arter anevrizması ve trombozu akılda bulunması gereken bir durumdur. Kronik semptomlarda bifid medyan sinir hipertrofisi ve kalsifiye medyan arter, akılda bulunması gereken bir durumdur. Etiyolojiye yönelik tanı genellikle preoperatif renkli Doppler USG ve MRG ile doğrulanmalıdır. Medyan arter onarımının sinir beslenmesi ve sinir iyileşmesi üzerine etkisi tartışılması gereken bir durumdur.

Persistent Median Artery and Bifid Median Nerve: Case Report

Abstract

Generally bifid median nerve and persistent median artery anomaly is asymptomatic and mostly it is an intraoperatively diagnosed anomaly. Bifid median nerve is usual form. But bifid median nerve and median artery association can be seen rarely. In this article asymptomatic, available but intraoperatively diagnosed bifid median nerve and median artery association is presented. Bifid median nerve and median artery association can be seen in 1-2% of population. This anomaly has asymptomatic findings, but in some cases acute and chronic findings can be seen as well. Aneurysm and thrombosis of median artery is an example for acute symptoms. And bifid median nerve hypertrophy and calcified median artery can be given as an example for chronic symptoms. Etiological diagnosis must be verified by preoperative colored Doppler USG and by MRI. Effect of median artery repair on nerve supplying and on nerve healing requires argument.
