Current Approaches to Nutrition in Lipedema

Öz

Lipedema is a fatty tissue disease whose etiology and pathophysiology are not clearly known. It was first described by Allen and Hines in 1940. Four different types of treatment are applied: medical, physical, surgical and medical nutrition therapy. In nutritional treatment, types of diet that will reduce the patients' inflammation and rich in antioxidants are recommended. One reason for this is that some genes in the blood and lymphatic vessels are thought to play a role in the development of the disease and these genes cause hyperplasia. Another theory is that the disease is caused by estrogen and occurs as a result of a disorder in the synthesis of the estrogen hormone. The quality of life of individuals with lipedema decreases due to the excess fat tissue in the lower extremities and the result of an increase in the severity of pain. Medical nutrition therapy and physical therapy play an important role in improving patients’ quality of life and enabling them to maintain their daily activities. That's why in many case studies, Mediterranean diet, gluten-free diet, ketogenic diet, and high-fat low-carb diet were applied to reduce inflamed tissue in patients. In this study, the changes in blood parameters and body composition of patients with lipedema due to diet types and nutritional supplements were summarized.

Lipödemde Beslenmeye Güncel Yaklaşımlar

Öz

Lipödem, etiyolojisi ve patofizyolojisi net olarak bilinmeyen bir yağ doku hastalığıdır. İlk kez Allen ve Hines tarafından 1940 yılında tanımlanmıştır. Medikal tedavi, fizik tedavi, cerrahi tedavi ve tıbbi beslenme tedavisi olmak üzere 4 farklı tedavi çeşidi uygulanmaktadır. Beslenme tedavisinde hastaların inflamasyonunu azaltacak antioksidandan zengin diyet tipleri planlanmalıdır. Bunun sebeplerinden biri hastalığın oluşmasında kan ve lenfatik damarlarda bazı genlerin rol oynadığı ve bu genlerin hiperplaziye yol açtığının düşünülmesidir. Bir diğer teoriyse hastalığın östrojen kaynaklı olduğu ve östrojen hormonunun sentezindeki bozukluk sonucu oluştuğuna dayanmaktadır. Lipödemi olan bireylerin yaşam kalitesi, alt ekstremitede yağ dokusunun fazla olması ve buna bağlı olarak ağrı şiddetinin artması nedeniyle düşmektedir. Hastaların yaşam kalitelerinin artması, günlük yaşamdaki faaliyetlerini sürdürebilmesi için tıbbi beslenme tedavisi ve fizik tedavi önemli bir rol oynamaktadır. Bu yüzden birçok vaka çalışmasında hastaların iltihaplı dokusunu azaltmak için Akdeniz diyeti, glutensiz diyet, ketojenik diyet ve yüksek yağlı düşük karbonhidratlı diyet uygulanmıştır. Bu çalışmada diyet türleriyle besin takviyelerinin lipödemli hastalardaki kan parametreleri ve vücut kompozisyonlarındaki değişimler özetlenmiştir.
