Pediatrik Hastalarda Üreter Taşı Tedavisinde Üreterorenoskopi Sonuçlarımız

Abstract

Bu çalışmada, ikinci basamak sağlık merkezinde çocuk hastalarda uygulanan üreterorenoskopi (URS) ile ilgili sonuçlarımızın sunulması amaçlanmıştır. Şubat 2011 ve Ocak 2014 tarihleri arasında kliniğimizde URS uygulanan, yaşları 18 ve altı olan, toplam 48 hastanın dosyaları retrospektif incelendi. Gerekli laboratuar ve radyolojik tetkikler sonrasında cerrahi yapılacak hastalara URS uygulandı. Operasyon sonrası radyolojik tetkikler ile rezidü taş varlığı araştırıldı. Operasyon süresi, peroperatif veya postoperatif komplikasyon varlığı, üreteral stent kullanımı değerlendirildi. Üreter taşı tanısı ile URS yapılmış toplam 48 hastanın 19’i kız, 29’u erkek ve yaş ortalaması 13.4(1-18) idi. Ortalama taş boyutu 8.7 mm (5-12 mm) olarak saptandı. Beş hastada (%10.4) proksimal üreterde, diğerlerinde orta veya distal üreterde taş saptandı. İşlem sonrasında taşsızlık %85.4 olarak bulundu. Dört hastaya (%8.3) üreteral çift j stent çekimi sırasında tekrar URS yapıldı ve taşsızlık sağlandı. Böbreğe taş migrasyonu ise üç hastada (%6.3) oldu. Operasyon süresi ortalama 48 dakika (30-90 dakika) olarak saptandı. Peroperatif majör komplikayon gözlenmedi. İşlem sonunda, 30 hastaya (%62.5) üreteral çift j stent ve 10 hastaya (%20.8)üreter kateteri yerleşirildi. Postoperatif dört hastada (%8.3) ateş saptandı. Postoperatif ortalama hastanede kalış süresi 1.4 gün (1-7 gün) olarak saptandı. Ülkemizde genellikle üçüncü basamak sağlık merkezlerinde yapılmasına karşın deneyimli ikinci basamak merkezlerde de pediatrik hasta grubunda URS’nin yapılabileceği kanaatindeyiz.

Our Results of Ureterorenoscopy in the Treatment of Pediatric Ureteral Calculi

Abstract

The aim of this study was to evaluate there sults of ureterorenoscopy (URS) that performe in a regional state hospital. We retrospectively analyzed 48 URS performed cases that treated at our clinic between February 2011 and January 2014. After necessary laboratory and radiological investigations, we performed URS for the patients those diagnosed as having stones needed surgery. After surgery, we investigated radiologically whether residual Stone fragments remained or not. Operative time, intraoperative or postoperative complications, ureteral stents were evaluated. Of the totally 48 patients (19 girls, 29 boys) underwent URS at our clinic. The mean age was13.4 (range: 1-18) and average stone size was 8.7 mm (5-12 mm), respectively. Five of the Stones were localized in the upper ureter and others were localized in the middle or lower ureter. Stone- free rate was 84.1%. In four patients (8.3%) repeat URS was performed to retrieve residual Stones during removal of the double J stents. Three of the stones (6.3%) migrated to the kidney. Mean duration of surgery was 48 min (30–90 min). No perioperative major complication was observed. Double J stents were inserted in thirty patients (62.5%) and ureteral catheterization was performed in ten patients (20.8%). The postoperative fever was observed in four (8.3%) patients. The mean postoperative hospital stay was 1.4 days (1-7 days), respectively. In our country, URS in pediatric age group is one of the surgeries which are mostly preferred to be performed in tertiary care centers. Our results supports the idea that this surgery can be performed by experienced surgeon seven in secondary care centers.
