Geriatrik Cerrahi Hastalarında Ağrı Değerlendirme Yöntemleri: Sistematik Derleme

Abstract

Amaç: Bu çalışma geriatrik cerrahi hastalarında ağrı değerlendirilmesinde kullanılan yöntemleri incelemek amacıyla yapılmıştır. Gereç ve Yöntem: Bu sistematik derleme için Pubmed, Sciencedirect, Google Scholar ve Ulakbim’de yer alan, “Geriatri”, “Ameliyat Sonrası”, “Ağrı Değerlendirmesi” anahtar kelimeleri kullanılarak 2018-2022 yılları arasında yapılmış Türkçe ve İngilizce araştırma makaleleri incelendi. Sistematik derleme kapsamına alınma kriterleri arasında; 2018-2022 yılları arasında yayınlanan, Türkiye ve yurtdışında yapılmış, geriatrik cerrrahi sonrası ağrı değerlendirmesinde kullanılan yöntemleri kapsayan, inceleme sonucu derleme kapsam kriterlerine uyan 15 makale çalışmaya alındı. Bulgular: Literatür taraması sonucu çalışma kriterlerine uyan 15 makale çalışma kapsamına alındı. Çalışmada incelenen araştırmaların en çok (%40’ı) 2020 yılında yayınlandığı, araştırma desenine göre %53,3’ünün (n=8) tam deneysel çalışma tasarımına sahip olduğu saptandı Ağrı değerlendirilmesi yöntemi olarak en çok (%43,8’i) Görsel Analog Skalası (VAS) kullanıldığı belirlendi. Sonuç: Bu sistematik derlemede, geriatrik hastaların, fizyolojik ve bilişsel bozuklukları olma potansiyellerinden dolayı, ağrı duygusunu deneyimleme konusunda dezavantajlı bir grup olarak değerlendirilmeleri gerektiği, ancak ağrı değerlendirmelerinin yetersiz ve uygun olmayan araçlarla yapıldığı belirlendi. Geriatrik cerrahi hastalarının ağrı değerlendirme yöntemlerini inceleyen kanıta dayalı bilgileri arttıracak daha fazla randomize kontrollü klinik çalışmaların yapılması önerilmektedir.
