Mesane tümörlerinde klinik tanısal belirteç olarak NMP-22 mesane kontrol testi

Abstract

Amaç: Bu çalışmada NMP-22 mesane kontrol testinin primer mesane tümorü tanısında ve takibindeki etkinliği araştırıldı.Gereç ve Yöntemler: Çalışmaya yaşları 49 ile 90 arasında ortalama yaş 62.2 toplam 132 hasta dahil edildi. Grup 1 n=66 hastalar, üroloji polikliniğine başvuran makroskopik hematüri, sistizm ve/veya prostatizm gibi mesane tümörü bakımından yüksek riskli hastalardı. Grup 2 n=66 hastalar ise görüntüleme yöntemi ile ve sistoskopik inceleme esnasında mesane tümörü tanısı alan hastalardı. Her iki gruptaki hastalardan alınan idrar örneklerinde NMP22 mesane kontrol testi uygulanarak teste ait duyarlılık, özgüllük, ve negatif-pozitif öngörü değerleri belirlendi. Ayrıca her iki grupta mesane tümörü saptanan olgularda; tümör derecesi, tümör evresi, tümör boyutu ve tümörün makroskopik görünümü gibi tümör özellikleri ile NMP-22 mesane kontrol testi pozitifliği karşılaştırıldı.Bulgular: NMP-22 mesane kontrol testine ait duyarlılık, özgüllük, negatif ve pozitif öngörü değerleri sırasıyla %67.6, % 86.6, %85.7 ve %69.7 olarak saptandı. Tümör derecesi grade ile test pozitifliği arasındaki bağlantı dışında diğer parametreler açısından istatistiksel olarak anlamlı bir farklılık saptanmadı. Sonuç: NMP-22 mesane kontrol testinin primer mesane tümör tanısında sistoskopi ve mesane biyopsisine alternatif olamayacağı görülmüştür. Bununla birlikte seçilmiş hasta grubunda takiplerde sistoskopi sıklığının belirlenmesi amacıyla kullanılabilir

NMP-22 bladder check test as a clinical diagnostic marker for bladder tumors

Abstract

Objectives: In this study, the effect of NMP-22 check test on the diagnosis and progress of primary bladder tumors was evaluated. Material and Methods: A total of 132 patients ranging in age from 49 to 90 years mean age 62.2 years were included to the study. Group 1 n=66 subjects were the patients who admitted to the urology outpatient clinic with macroscopic hematuria, cystitism and/or prostatism symptoms highly suggestive of bladder tumor. Group 2 n=66 patients, were subjects in whom bladder tumor was detected via imaging methods and whose bladder tumor was seen during the cystoscopic examination. NMP-22 bladder check test was performed both groups’ urine samples for test’s sensitivity, specifity and positive-negative predictive value. Also determined bladder tumor in both groups; tumor grades, stages, size, macroskobic appearance belonging to bladder cancer detected patients were compared with the outcomes of NMP22 Bladder Check Test. Results: NMP-22 Bladder Check Test’s sensitivity, specifity, positive predictive value, negative predictive value are determined %67.6, % 86.6, %85.7 and %69.7 respectively. Except the relation between tumor grade and positive results of the test, there were not any statistically significant difference between the other parameters.Conclusion: It was seen that, NMP-22 Bladder Check Test could not be an alternative to cystoscopy and bladder biopsy, in the diagnosis of primary bladder tumor. However, progress of selected patient groups, it can be used to determine the frequency of cystoscopy
