Böbrek taşı perkütan cerrahisinde interkostal ve subkostal yaklaşımın başarı ve komplikasyon oranlarının karşılaştırılması

Abstract

Amaç: Çalışmada böbrek taşı sebebiyle perkütan nefrolitotomi yapılan hastalarda kullanılan interkostal ve subkostal giriş tekniklerinin başarı ve komplikasyonları karşılaştırıldı.Gereç ve Yöntemler: Nisan 2006 – Ocak 2009 tarihleri arasında Ankara Dışkapı Yıldırım Beyazıt Eğitim ve Araştırma Hastanesi 4.Üroloji Kliniğinde perkütan nefrolitotripsi PCNL yapılan hastalar arasından üst ve orta kaliks, stoghorn, kompleks alt kaliks taşları olan ve giriş olarak subkostal triangulasyon alt kaliks girişi ya da interkostal orta veya üst kaliks girişi kullandığımız 165 hastanın verileri retrospektif olarak değerlendirildi. Subkostal giriş yapılan hastaları Grup1 118 , interkostal giriş yapılanları grup2 47 olarak ayrıldı. Bulgular: Hastaların yaş ortalamaları grup 1 44±16,3 ve grup 2’de 44,1±13,3 p=0,98 benzerdi. Taş boyutu grup 1’de 323,38±169,72 mm2, grup 2’de 310,85±161,28 mm2 p:0,73 operasyon süresi sırasıyla 118,9±46,8 dk ve 122,1 ±49,5 dk p:0,32 skopi süreleri 5,10±3,25 dk ve 6,56±5,06 dk p:0,18 olarak hesaplandı ve istatistiksel açıdan anlamlı saptanmadı. Hastaların postoperatif ilk 6 saat içerisindeki hematokrit değerlerindeki azalma grup 1’de 6,46±3,94 mg/dl, grup 2’de ise 5,86±4,4 mg/dl p:0,12 olarak saptandı ve istatistiksel olarak anlamlı değildi. Re-entry çekilme zamanı sırasıyla 3,03±2,41 gün ve 3,39±2,6 gün p:0,10 , rezidü taş boyutu grup 1’de 102.73±26,4 mm2, grup 2’de 118±34,6 mm2 hesaplandı p:0,11 ve gruplar benzer çıktı. Hastaların hastanede yatış süreleri sırayla 4,53±3,8 gün ve 5,08±3,1 gündü p=0,05 istatistiksel olarak fark saptanmadı. Hastalarımızın hiçbirinde pnömotoraksa rastlanmadı. Sonuç :İnterkostal girişin özellikle 11.-12. interkostal aralık seçildiğinde önemli bir komplikasyon olan akciğer yaralanması açısından subkostal giriş kadar güvenli olduğunu, bunun yanında taşsızlık oranı, kanama miktarı, operasyon ve skopi süresi, hospitalizasyon süresinin de benzer olduğunu saptadık

Comparison of success and complication rates of intercostal and subcostal approach in percutaneous nephrolithotomy

Abstract

Objective: In this study, we aimed to compare the success and complication rates of intercostal and subcostal aproach in percutaneous nephrolithotomy. Material and Methods: We evaluated the data of 165 patients operated at Diskapi Yildirim Beyazit Training and Research Hospital Department of Urology Clinic between April 2006-January 2009 for isolated upper-mid pole caliceal group stones, staghorn kidney stones and complex lower calicial stones with subcostal triangulation or intercostally. Subcostal triangulation patients were grouped as 1 n:118 and intercostal cases were group 2 n: 47 .Results: Mean ages of group 1 and group 2 were 44±16,3 and 44,1±13,3 respectively. Mean duration of operation time was 118,9±46,8 min and 122,1±49,5 min p=0,32 . Scopy times were 15,10±3,25 min and 6,56±5,06 min p=0,18 . The post op 6 hours heamothocrit loss was 6,46±3,94mg/dl and 5,86±4,4mg/dl p=0,12 for group 1 and 2 respectively. Reentry malecott removal time was, 3,03±2,41 days and 3,39±2,6 days p=0,10 . Residual stone sizes were 102.73±26,4 mm2 and 118±34,6 mm2 for both groups respectively p=0,11 . Duration of hospitalization was 4,53±3,8 days and 5,08±3,1 days p=0,05 respectively. Pneumothorax and heamothorax complications were not present in either groups.Conclusions: Intercostal access is as safe as the subcostal access when the access is performed between 11 and 12th intercostal spaces süreleri sırayla 4,53±3,8 gün ve 5,08±3,1 gündü p=0,05 istatistiksel olarak fark saptanmadı.
