Perkütan nefrolitotomide preoperatif hidronefroz derecesinin operasyon süresine ve kanamaya etkisi

Abstract

Amaç: Perkütan nefrolitotomide preoperatif hidronefroz derecesinin operasyon süresine, kılavuz teli üretere atma süresine, floroskopi süresine, irrigasyon sıvısı kulanım miktarına, taşsızlığa ve kanamaya olan etkisini araştırdık.Gereç ve Yöntemler: Ocak 2009 – Aralık 2012 tarihleri arasında hastanemiz üroloji kliniğinde toplam 405 hastaya PNL operasyonu uygulandı. Hidronefrozu olmayan olgular Grade 0 Grup 1’e, düşük dereceli hidronefrozu olan olgular ise Grup 2’ye, ileri derece hidronefrozu olan olgular Grup 3’e dahil edildi. Bu çalışmada üç grubun operasyon süreleri, uygun access sağlanıp vizyon oluşturulana kadar geçen süre, floroskopi süreleri, operasyon öncesi ve sonrası hematokrit değerlerindeki değişim, taşsızlık oranları açısından karşılaştırıldi.Bulgular: Hasta sayıları Grup 1’de 120, Grup 2’de 160, Grup 3’de ise 125 idi. Gruplardaki hastaların yaş ortalaması Grup 1 de 47,76±15,14 yıl, Grup 2 de 42,31 ±15,06 yıl ve Grup 3’de ise 49,05±11,50 yıl olarak hesaplandı. Bütün gruplar daha önce geçirilmiş operasyon yönünden, giriş şekli, taş lokalizasyonu ve taş yükü açısından benzerdi.Floroskopi süresi Grup 1’de 5,45±3,74 ,Grup 2’de 6,14±4,39 ve Grup 3’de 6,41±4,08 dk olarak hesaplandı ve aradaki fark anlamlı olarak saptanmadı. Kılavuz teli üretere atma süresi ise Grup 1’de 8,63±5,79, Grup 2’de 5,58±3,5 ve Grup 3’de 3,32±1,82 dk olarak hesaplandı ve aradaki fark anlamlı olarak saptandı p

Effect of hydronephrosis on operative time, success and duration upon percutaneous nephrolitotomy

Abstract

Objective: We aimed to investigate the effect of hydronephrosis on operative duration, dilatation duration, fluoroscopy time, stone free state and bleeding upon percutaneous nephrolitotomy PNL . Materials and Methods: Data of 405 patients underwent PNL in our clinic between January 2009 and December 2012. The patients were divided into three groups as no hydronephrosis Group 1 , low degree hydronephrosis Group 2 , high degree hydronephrosis Group 3-4 . Three groups were compared according to operative parameters. Results: Group 1 consisted of 120, group 2 consisted of 160 and group 3 consisted of 125 patients. Mean patient age was 47,76±15,14, 42,31 ±15,06 and 49,05±11,50 for Groups 1,2 and 3 respectively. All groups had similar previous operation history, access technique, stone localization and stone load. Flouroscopy times were 5,45±3,74 ,6,14±4,39 and 6,41±4,08 minutes for Groups 1, 2 and 3 respectively.
