Penis fraktürü ve tedavi sonuçları, tek merkezde 46 olgunun retrospektif değerlendirilmesi

Abstract

Amaç: Penil fraktür etiyolojisi, acil cerrahi tedavi ve postoperatif komplikasyonlarının retrospektif olarak değerlendirilmesi.Gereç ve Yöntemler: Bu çalışmamızda, mayıs 2006 ve mayıs 2013 yılları arasında acil cerrahi girişim ile tedavi ettiğimiz ve takibi mümkün olan 46 olgu retrospektif olarak değerlendirildi. Hastaların tümünde tanı anamnez ve fizik muayene ile konuldu. Ek görüntüleme tekniklerine ihtiyaç duyulmadı. Hastalar acil serviste muayene edildikten en geç 4 saat içinde operasyona alındılar. Hepsinde subkoronal sirkumsizyonel insizyonla girilerek penis derisi soyuldu ve tunikadaki yırtıklar 3/0 vicryl ile primer tamir edildi. Ek olarak üretral defekti olanlar foley sonda klavuzluğunda primer olarak onarıldı.Bulgular: Hastaların yaş ortalaması 33,2±9,4 idi. Hastaların fraktür sonrası hastaneye başvuru süreleri ortalama 4,97±0,31 saatte değişiyordu. Etyolojik olarak en sık sebebin 22 olgu ile cinsel ilişki sırasında olduğu görüldü. Acil olarak operasyona alınan hastalarda ortalama defekt boyutu 1,75±0,62 cm olarak ölçüldü. Olguların 4’ünde penis fraktürüne ek olarak üretrada defekt eşlik ediyordu. Hastalar operasyon sonrası 3. ayda, 6. ayda, 12. ayda kontrole çağrıldı. Hastaların hiç birinde penil deformite saptanmadı. 6. ayda ve 12. ayda yapılan sorgulamalarda IIEF-5 skorlarının ortalama 22,6±0,79 olduğu izlendi. Üretral travması olan 4 hastada üretral darlık gelişmediği görüldü.Sonuç: Penil fraktür tipik fizik muayene ve anamnezi ile kolay tanı koyulabilen nadir bir ürolojik acildir. Hastalar hızlı bir şekilde değerlendirilmeli ve mümkün olan en kısa sürede ameliyata alınmalıdır. Acil cerrahi sonrası erken ve geç dönem sonuçları oldukça iyidir

Penile fracture and results of treatment, retrospective study of 46 patients in single center

Abstract

Objective: Assessment of penile fracture etiology, immediate surgical treatment and postoperative complications retrospectively.Material and Methods: In this study, between May 2006 and May 2013 possible follow-up 46 patients, have treated with emergency surgery, were evaluated retrospectively. In all patients, the diagnosis was made by history and physical examination. And did not need additional imaging techniques. Patients were included in the operation no later than 4 hours after the examination in emergency department. In all of them, penis skin opened with subcoronal circumcision incision and lasarations of tunica were fixed primarily with 3/0 vicryl suture. In addition, patients with urethral defect were primarily fixed with foley catheter guidance.Results: The average age of the patients was 33,2±9,4. The average length of hospital admissions of patients after fracture changes in 4,97±0,31 hours. The most common etiologic cause was found to be during sexual intercourse in 22 of 46 cases. The mean size of the defect was measured as 1.75 ± 0.62 cm in patients who went to an emergency operation. In 4 of 46 cases,in addition to penil fracture was accompanied by a urethral defect. Patients were recalled to control at the 3rd, 6th, and 12th months after the operation. None of the patients had penile deformity. The avarage IIEF-5 scors were 22,6±0,79 at 6th and 12th görüldü.Sonuç: Penil fraktür tipik fizik muayene ve anamnezi ile kolay tanı koyulabilen nadir bir ürolojik acildir.
