Priapizm tedavisinde Al-Ghorab tekniğinin etkinliği

Abstract

Amaç: Aspirasyon ve medikal tedavi ve aspirasyon tedavisinin başarısız olduğu durumlarda şant teknikleri tanımlanmıştır. Distal ve proksimal tekniklerin tanımlandığı; özellikle de distal tekniklerin popüler olduğu bu günlerde, bir açık teknik olarak Al-Ghorab yöntemini uyguladığımız hastaları ve orta dönem sonuçlarımızı sunmayı amaçlamaktayız.Gereç ve Yöntemler: Haziran 2008- Haziran 2012 tarihleri arasında hastanemizin acil servisine priapizm şikayeti ile başvuran hastalar arasında 15 hastanın dosyası geriye dönük olarak incelendi. Hastalar öncelikle kavernoz cismin boşaltılması ve sempatomimetik ajan uygulaması epinefrin ile tedavi edilmiş ve bu aşamadan sonra hastalara genel anestezi altında müdahale yapılmıştı. Hastaların priapizm süreleri, post operatif yaşam kaliteleri, vizüel analog skala VAS ile pre ve post operatif ağrıları, Uluslararası Erektil Fonksiyon Endeksi -5 İEEF-5 ile erektil fonksiyon durumları değerlendirildi.Bulgular: Hastaların yaş ortalaması 45.2 idi . Ortalama priapizm süresi 16.2 saat idi. Preoperatif VAS değerleri ortalama 6,9 iken, post operatif VAS seviyeleri 2,3 idi. Hastalar ortalama olarak 12 ay boyunca takip edilmişti. 12. ay sonunda İEEF ortalama 19,4 idi.Sonuç: Priapizmin cerrahi tedavisinde, AlGhorab yönteminin invaziv olmasının yanında güvenilir ve etkili bir yöntem olduğunu düşünmekteyiz

The efficacy of Al-Ghorab technique in the treatment of priapism

Abstract

Objective: It is described that shunt techniques are the treatment options when aspiration and medical treatments were failed. Especially, the distal shunt techniques are popular in nowadays. The aim of the study is to demonstrate mid-term outcomes of the distal surgical technique of priapism, Al-Ghorab approach.Material and Methods: Fifteen patients applied to our emergency clinic between June 2008 and June 2012 with the diagnosis of priapism were included in the study. Patients were initially treated by aspiration and intracavernosal sympathomimetic epinephrine application. Then Al-Ghorab technique was applied under general anesthesia to these patients. Patients were further evaluated with duration of priapism, post-operative quality of life, visual analogue scale VAS , and international index of erectile function IIEF-5 . Results: Mean patient age was 45.2 . Mean priapism duration was 16.2 hours . Preoperative VAS scores was 6,9 and postoperative VAS scores was 2,3 . After 12 months of follow op, IIEF scores were found to be 19,4 .Conclusion: Even though Al-Ghorab is an invasive approach, we believe that it is a safe and efficient technique
