Tek taraflı tedavi edilmemiş inguinal testis hastalarında endokrin fonksiyonlar ve spermatogenez

Abstract

testisi bulunan genç erişkin dönemindeki kişilerin endokrin testiküler fonksiyonlarının ve spermatogenez durumlarının araştırılması. Gereç ve Yöntemler: Polikliniğe başvuran diğer testis normal olmak üzere tek taraflı inguinal yerleşimli testisi bulunan 18 yaş üzeri genç erişkin erkekler sayı=14 olgu çalışmaya alındı. Bu olguların hasta kayıtları kullanılarak spermiyogram değerleri sayı=8 olgu ile toplam testosteron, luteinizan hormon LH ve folikül stimülan hormon FSH düzeyleri sayı=11 olgu retrospektif olarak değerlendirildi. Bulgular: Çalışmaya yaş ortalaması 21.92 ± 2.23 yıl olan 14 hasta alındı. Hastaların büyük kısmı primer hasta iken bir hasta 9 yaşında testisinin inguinale yerleştiğini belirtmiş ve bir hastada 5 yaşında başarısız orşiopeksi uygulandığını belirtmiştir. Hastaların 9 tanesinde sağ, 5 tanesinde sol inguinal testis saptandı. Olguların ortalama testosteron, LH, FSH değerleri sırasıyla 4.42±1.24 ng/ml; 7.56±4.1mIU/ml; 8.59±6.68mIU/ml olarak bulundu. Spermiogram sonuçlarında ise sadece bir olguda azoospermi saptanırken ortalama ejekulat hacimleri 1.93±1.2ml, mililitrede ortalama sperm sayısı 53.8±41.3 milyon, ortalama normal morfolojideki sperm yüzdesi 78.1±17.1 ve ortalama hareketli sperm yüzdesi a+b 41.4±24.2 olarak bulunmuştur. Sonuç: Diğer testis normal olmak üzere tek taraflı inguinal testis varlığı belirgin sperm kalitesi bozukluğu oluşturmaktadır.

Endocrine functions and spermatogenesis in patients with non-treated unilateral inguinal testis

Abstract

Objective: To analyze status of endocrine functions and spermatogenesis in adult patients with non-treated unilateral inguinal testis. Material and Methods: A total of 14 adult men with unilateral testis were studied. We evaluated sperm quality n=8 and total testosterone, luteinizing hormone, follicular stimulating hormone levels n=11 retrospectivly. Results: Mean age was 21.92 ± 2.23 years. Ascending testis at 9 years old was reported in one patient and non successful orchiopexy at 5 years old was reported in one patient. Other patients that studied were primary cases. Nine of the cases were right sided and five were left. Mean total testosterone, luteinizing hormone, follicular stimulating hormone levels were 4.42±1.24 ng/ml;7.56±4.1mIU/ml; 8.59±6.68mIU/ml . Azoospermia was observed in only one patient. Mean sperm volume was 1.93±1.2ml, mean of sperm concentration was 53.8±41.3 sperm/mL x106 , mean percent of normal sperm morphology was 78.1±17.1% and mean percent of rapid and slow progressive motility a + b was 41.4±24.2%. Conclusions: There were abnormality in sperm quality in patients with unilateral inguinal testis and normal contralateral testis.
