Tüm anterior üretrayı tutan darlıkların tedavisinde tek aşamalı dorsal inlay bukkal mukozal greft üretroplasti

Abstract

Amaç: Tüm anteriyor üretrayı tutan darlıkların tedavisinde tek aşamalı dorsal inlay bukkal mukozal greft BMG üretroplasti yöntemiyle ilgili sonuçlarımızı değerlendirmek.Gereç ve Yöntemler: Kliniğimizde Aralık 2010 ve Aralık 2011 tarihleri arasında anteriyor üretra darlığı nedeniyle üretroplasti yapılan 18 erkek hasta değerlendirildi. Bu hastalar arasında tüm anteriyor üretrayı tutan, tek aşamalı dorsal inlay BMG yapılan 6 hasta çalışmaya alındı. Hastalar operasyon öncesi retrograd üretrografi RÜ , işeme sistoüretrografisi İSÜG ve üretrosistoskopi ile değerlendirildi. Tüm hastalar postoperatif 3. hafta ve 3. ayda RÜ+İSÜG, idrar akım hızı ve işeme sonrası artık idrar miktarı AİM ile izlendi, sonrasında takip senelik yapıldı. Cerrahi başarı, son izlemdeki idrarda tıkanıklık bulgularının olmaması, ek girişim gerektirecek darlık saptanmaması ve hastanın subjektif memnuniyeti olarak tanımlandı.Bulgular: Ortalama hasta yaşı 43,5 ±11,5 idi. Ortalama BMG uzunluğu 13 cm 11-15 cm , ortalama operasyon süresi 170 dakika 120-200 dak , kan kaybı ise 60 ml olarak hesaplandı. Ortalama hastanede kalış süresi 3,7 gün idi. Hiçbir hastada intraoperatif veya postoperatif komplikasyon gelişmedi. En az 1 yıllık aralık: 12-26 ay izlemde tüm hastalarda objektif ve subjektif başarı elde edildi. Postoperatif en son izlemdeki “Qmax” değerlerinde, preoperatif değerlere göre istatistiksel anlamlı düzelme olduğu görüldü ortalama 24,5±8,3 ml/sn’ye karşılık 6,2±5,8 ml/sn, p=0,001 . Sonuç: Tek aşamalı dorsal inlay BMG üretroplasti, yaygın anteriyor üretra darlığı bulunan seçilmiş olgularda uygun bir tedavi yöntemi olarak gözükmektedir

One-stage dorsal inlay buccal mucosal graft urethroplasty in the management of extensive anterior urethral strictures

Abstract

Objectives: We evaluated the efficacy of one-stage dorsal inlay buccal mucosal graft BMG urethroplasty in the management of extensive anterior urethral strictures. Materials and Methods: Between December 2010 and December 2011, 18 patients with anterior urethral strictures underwent urethroplasty in our clinic. Six of 18 patients who received one-stage buccal mucosal greft urethroplasty due to extensive anterior urethral stricture were included in this study. Patients were evaluated with preoperative retrograd uretrography RU , voiding cystourethrography VCUG , and urethrocystoscopy. All patients received RU+VCUG, uroflowmetry and postvoid residual urine determination at postoperative 3rd week and 3rd month, then followed yearly thereafter. Cure was defined as no evidence of obstruction on uroflowmetry, the absence of any strictures that may require additional intervention and subjective patient satisfaction at the last visit. Results: Mean patient age was 43.5 years. The mean length of BMG was 13 cm 12-16 , mean operation time was 170 130-240 min, and estimated blood loss was 60 30-110 ml. Mean hospitalization was 3.7 days. There were no intraoperative or postoperative complications. With at least one year of follow-up range=12-26 months , objective and subjective success was achieved in all patients. Mean Qmax was significantly improved at the last follow-up as compared to preoperative measurements 24.5±8.3 ml/s vs. 6.2±5.8 ml/s, p=0.001 . Conclusion: One-stage dorsal inlay BMG urethroplasty seems as a feasible option in selected cases who have extensive stenosis of the anterior urethra.
