Hangi prostat spesifik antijen değeri önemli?

Abstract

Amaç: Biz bu çalışmamızda kliniğimizde transrektal ultrason TRUS kılavuzluğunda yapılan prostat biyopsilerinde prostat spesifik antijen PSA sonuçlarına ve serbest PSA/ total PSA oranlarına göre prostat kanseri saptama oranlarını, Gleason gradelerini, malignite izlenen kor sayılarını ve malignite saptanan korların ortalama malignite yüzdesini paylaşarak biyopsi endikasyonları konusundaki belirsizliğin giderilmesine katkıda bulunmak istedik. Gereç ve Yöntemler: Ocak 2012- Kasım 2013 arasında ortalama yaşları 66 45-75 olan TRUS eşliğinde biyopsi yapılan 184 hastanın işlem öncesi ölçülen PSA değerleri ile patoloji sonuçları retrospektif olarak değerlendirildi. Hastalar PSA değerleri 2.5-4 ng/mL, 4.1-10 ng/mL ve >10 ng/mL olacak şekilde grup 1, 2 ve 3 ve PSA değerleri 2.5-10 ng/mL arasında olanların serbest/total PSA değerleri hesaplanarak serbest/ total PSA değerleri 0.25 olacak şekilde grup A, B ve C 3 gruba ayrıldı. Serbest/ total PSA oranları ve PSA değerlerine göre klinik önemli PKA’lar prostat kanseri istatistiksel olarak değerlendirildi. Bulgular ve Sonuç: PSA değerlerine göre ayrılan 1. ve 3. grupta elde edilen veriler %25 ve %51.2 daha önceki çalışmalarla paralel malignite saptama oranlarına sahipti. 2. grupta elde edilen %17.1 kanser saptama oranının ise önceki çalışmalara göre daha düşük olduğu görüldü. Serbest/total PSA değerlerine göre 0.1’in altı ve 0.1-0.25 arası değerlerde malignite saptama oran ları %33.3 ve %20.6 daha önceki çalışmalara paralel olarak 0.25’in üzeri değerlerde malignite saptama oranına %8.3 göre daha yüksek olarak bulundu. Çalışmamızda hesaplanan ve klinik önemli PKA’ların saptanmasında kullanılabilecek olan serbest/total PSA 0.12 cutoff değeri daha geniş serilerle desteklenmelidir. İlk olarak çalışmamızda kullandığımız ortalama PKA yüzdesinin ise yine klinik önemli PKA’ların saptanmasında kullanılan ve tümörün klinik seyri hakkında bilgi verebilecek olan tümör volümü hakkında öngörü sağlayabileceği düşünüldü

Which prostate spesific antigen level is important?

Abstract

Objective: We aimed to present our transrectal ultrasound TRUS guided biopsy results in correlation with prostate specific antigen PSA , free/total PSA levels, cancer detection rates, Gleason scores, cancer ratios of total cores and percentages in these cores, thus providing information to the literature. Material and Methods: 184 patients with a mean age of 66 45-75 underwent TRUS guided biopsy between January 2012 and November 2013. The PSA levels were compared with pathological results retrospectively. Patients were grouped according to their PSA levels; PSA levels between 2.5 and 4 ng/ml Group 1 , between 4.1 and 10 ng/ml Group 2 , and >10 ng/ml Group 3 . Patients with PSA levels between 2.5 and 10 ng/ml subdivided into 3 groups according to their free/total PSA levels; 0.25 Group A,B,C . Prostat cancer PCA patients were evaluated with PSA levels and free/total PSA ratios. Results and Conclusions: In the Group 1 and 3, malignancy rates %25 and %51.2 were paralel to the literature. In the Group 2 malignancy rate was %17.1, and lower than the results in the literature. In Group A and B, malignancy rates %33 and %20.6 were similar to the literature, while in Group C it was higher %8.3 . We found that the cutoff value of free/total PSA ratios at 0,12 has a predictive value to dedect clinical important PCA. We suggest that this cutoff value to predict clinical important PCA should be studied in larger randomize controlled series. The prostate cancer ratios in the prostatic biopsy specimen which is firstly used in our study was correlated with the clinically important prostate cancer rates, and tumor volume.
