Kadın stres üriner inkontinansında TVT ve TOT yöntemlerinin etkinliğinin ve güvenilirliğinin karşılaştırılması

Abstract

Amaç: Kadın stres üriner inkontinans olgularının tedavisinde kullanılan transobturator tape TOT ve tension free vajinal tape TVT operasyonlarının başarısını, komplikasyonlarını ve yaşam kalitesi üzerine etkilerini karşılaştırmak. Gereç ve Yöntem: Nisan 2007-Temmuz 2010 tarihleri arasında stres üriner inkontinans saptanan 162 hasta çalışmaya dahil edildi. Tüm hastalardan detaylı anamnez alındı ve fizik muayene, stres testi, Q tip test ve üriner ultrasonografi yapıldı. İnkontinans ve hayat kalitesi anketleri [Incontinence impact questionnaire IIQ-7 ve Urinary distres inventory UDI-6 ] dolduruldu. Çalışmaya alınan hastaların 75 tanesine TVT yöntemi ve 87 tanesine TOT yöntemi uygulandı. Ameliyatlardan ortalama 12 ay sonra stres testi, Q tip test, üriner ultrasonografi, inkontinans ve hayat kalitesi anketleri yapılarak operasyon öncesi değerlerler karşılaştırıldı. Bulgular: Tüm hastalar genel anestezi ve/ veya spinal anestezi altında opere edildi. İki grubun demografik veriler arasında fark yoktu. TOT grubunda operasyon süresi 15,3±4,2 dakika ve TVT grubunda ise 24,3±8,6 dakika tespit edildi ve istatistiksel olarak TOT grubunda operasyon süresi daha kısaydı p

Comparison of effectiveness and safety of TVT and TOT procedures in the treatment of female stress urinary incontinence

Abstract

Objective: To compare the success, complications and effects on life quality of transobturator tape TOT and tension free vaginal tape TVT procedures for female stress urinary incontinence. Materials and Methods: Between April 2007 and July 2010, 162 patients who diagnosed stress urinary incontinence were enrolled into study. All patients’ preoperative urological histories were taken, physical examination, stress test, Q tip test, urinary ultrasonography, incontinence and quality of life questionnairies [Incontinence impact questionnaire IIQ-7 and Urinary distres inventory UDI-6 ] were performed. Transobturator route was used in 87 patients and retropubic procedures were performed in 75 patients. Stress test, Q tip test, urinary ultrasonography, incontinence and quality of life questionnairies IIQ-7 and UDI-6 were repeated at 12 months after surgery and were compared with preoperative parameters. Results: All patients were operated under general and spinal anesthesia. There was no difference between the two groups in demographic data.
