Penil fraktür: Dört yıllık klinik deneyimimiz

Abstract

Amaç: Bu çalışmada kliniğimizde opere edilen penil fraktür hastalarının etyolojisi, preoperatif klinik bulguları ve postoperatif uzun dönem sonuçlarının değerlendirilmesi amaçlanmıştır. Gereç ve Yöntemler: Ağustos 2011 - Ağustos 2015 tarihleri arasında penil fraktür ön tanısı ile acil servise başvuran ve kliniğimizde opere ettiğimiz 25 hastanın verileri retrospektif olarak değerlendirildi. Hastaların tamamına penil fraktür tanısı fizik muayene ile konuldu. İki hastaya üretroraji sebebi ile pre-operatif üretrografi yapılmıştı. Hastaların 9’u genel anestezi ile opere edilirken 16’sı spinal anestezi altında opere edilmişti. Bulgular: Çalışmamızda hastaların ortalama yaşı 40.84±12.7 22-70 yıl olan 25 hastadan oluşmaktaydı. Bütün hastalar değerlendirildiğin de cinsel ilişki sıklığı 3.1±1.2/hafta olarak bulundu. Hastaların 13’i vajinal 2’si anal yolla olmak üzere 15’in de %60 penil fraktürün cinsel ilişki esnasında olduğu saptandı. Altı hastada %24 ise uyku sırasında dönme veya düşme, üç hastada %12 penil tümesans manevraları, bir hasta %4 ise mastürbasyon esnasında penil fraktür meydana gelmişti. Cinsel ilişki sırasında penil fraktür oluşan hastaların üçünde PDE5 inhibitörü %12 , üçünde ise kayganlaştırıcı kullanımı %12 mevcuttu. Sonuç: Penil fraktür hastalarının ayrıntılı pre-operatif değerlendirmesinde cinsel ilişki tipi, hastanın kullandığı ilaçlar, cinsel ilişki sıklığının sorgulanması tanı koymada ek bilgiler sağlayabilir. Penil fraktür tanısı tipik anamnez ve fizik muayene bulgularıyla kolaylıkla konabilir ve üretrorajisi olan hastalarda üretrografi gibi ek radyolojik tetkik gerekebilir.

Penil fracture: Four years clinical experience

Abstract

Objective: In this study we aim to review of etiology, pre-operative symptoms and postoperative long-term results of our operated penile fracture patients. Material and Methods: Data of 25 patients which applied to emergency room for penile fracture and operated in our clinic between August 2011- 2015 were retrospectively analyzed. All patients were diagnosed by physical examination. Two patients received preoperative urethrography because of urethrorrhagia. Nine patients were treated under general anesthesia, whereas 16 patients treated under spinal anesthesia. Results: Twenty-five patients mean age were 40.84±12.7 22-70 years old in our study. Sexual intercourse interval was 3.1±1.2/week of all patients datas reviewed. We evaluated cause of penile fracture in our patients; vaginal sexual intercourse in thirteen of patients and anal sexual intercourse in two patients totally 15 %60 patients had penil fracture caused by sexual intercourse. Six of patients %24 had fracture when sleeping or to fall in down, three of patients %12 had fracture caused by maneouver of penil tumescence and one of patient’s %4 fracture caused by masturbation. Three patients %12 of penile fracture caused by sexual intercourse had used PDE- 5% inhibitors and other three ones %12 had used gel . Conclusion: Detailed pre-operative evaluation of penil fracture patients reached additional information such as type of sexual intercourse, previous medications, interval of sexual intercourse. The diagnosis of penil fracture can reached easily with typical medical evaluation and physical examination and additional radiological tests such as urethrography need for penil fracture patients with urethrorrhagia
