Widespread Thrombus During Central Venous Catheterization in Ultrasonography: A Case

Abstract

Central venous catheterization CVC is commonly used for long-term treatment, venous access, parenteral nutrition, central venous pressure CVP monitoring and also used for hemodialysis and chemotherapy. Internal jugular vein and subclavian vein are the most commonly preferred veins. Nowadays, ultrasonography USG use has been increased. Realtime USG guided CVC placement increases the success of the process. In addition, the use of USG can assist the detection of pathological conditions which exist in vascular anatomical structures. Our case was 72 year old male patient, operated due to gastric carcinoma, received chemotherapy and radiotherapy and had difficult peripheral venous access. Widely disseminated thrombosis in the internal jugular vein was seen with USG and the lumen of IJV was found to be occluded. By the bilateral carotid doppler USG, 3.3 mm muralthrombus was diagnosed in the right common carotid artery and additionally widely disseminated thrombosis causing total occlusion extended inferiorly into the subclavian vein was reported. The USG guidance increases the success of CVC placement, reduces the risk of complications also increases patient comfort and safety. In conclusion: We think that the use of USG during CVC placement can help for detection of thrombus, prevention of complications and planning of treatment.

Ultrason Eşliğinde Santral Venöz Kateterizasyon Esnasında Fark Edilen Yaygın Trombüs: Bir Olgu

Abstract

Santral venöz kateterizasyon SVK , uzun süreli tedavi, venöz giriş, parenteral beslenme, santral venöz basınç SVB izlemi ve hemodiyaliz ve kemoterapi için yaygın olarak kullanılmaktadır. İnternal juguler ven İJV ve subklavyan ven, en sık tercih edilen damardır. Günümüzde ultrasonografi USG kullanımı artmıştır. Eş zamanlı USG kılavuzluğu SVK işlem başarısını artırır. Buna ekolarak, USG kullanımı, vasküler anatomik yapılarda varolan patolojik durumların saptanmasına yardımcı olabilir. Olgumuz: 72 yaşında erkek hasta, gastrik karsinom nedeniyle ameliyat edildi; kemoterapi ve radyoterapi aldı ve periferik venöz girişimi sıkıntılıydı. USG ile internal juguler vendeki yaygın trombüs görüldü ve IJV'nin lümeninin tıkandığı tespit edildi. Bilateral karotid doppler USG ile sağ ana karotid arterde 3,3 mm duvar trombozu teşhis edildi ve ekokardiyografide subklavyan venden aşağıya doğru genişlemiş total oklüzyona neden olan yaygın trombüs rapor edildi. USG rehberliği, SVK yerleştirilmesinin başarısını artırır, komplikasyon riskini azaltır, hasta konforunu ve güvenliği arttırır. Sonuç olarak, SVK yerleşimi sırasında USG kullanımının trombozun saptanmasında, komplikasyonların önlenmesinde ve tedavinin planlanmasında yardımcı olabileceğini düşünüyoruz.
