Prevalence of Obstructive Sleep Apnea Syndrome in Chronic Obstructive Pulmonary Disease

Abstract

Objective: Breathing disorders occured during sleep have been able to reach the severe dimensions and have led to an increase of morbidity and mortality in these patients. Detection of these disorders is very important in terms of both the prognosis and the application of an effective treatment. In this study, we aimed to research the prevalence of obstructive sleep apnea syndrome OSAS with the polysomnography in chronic obstructive pulmonary patients. Method: Diagnosed with chronic obstructive pulmonary disease COPD , 42 patients 21 female, 21 male were included to the study. Smoking history of the patients, the results of the blood gas analysis BGA , hematocrit values and anthropometric characteristics were recorded. Epworth Sleepiness Scale was used in order to evaluate the excessive daytime sleepiness objectively. Polysomnography was performed to all of the patients. Results: The rate of OSAS was found to be 61.9% in patients with COPD. 30.7% of the cases with OSAS were in mild, 42.30% were in moderate and 26.9% were in severe OSAS class. There was no statistical difference between the groups with and without OSAS in terms of the demographic characteristics, breathing function test BFT , BGA and hematocrit values. Snoring in 69% of the cases, the excessive daytime sleepiness in 61.9% and witnessed apnea in 59.5% were detected. Kronik obstrüktif akciğer hastalığında obstrüktif uyku apne sendromu sıklığı

Abstract

Amaç: Uyku sırasında oluşan solunum bozuklukları ciddi boyutlara ulaşabilmekte ve bu hastalarda morbidite ve mortalitenin artmasına yol açmaktadırlar. Bu bozuklukların saptanması, hem prognoz hem de etkili bir tedavinin uygulanması açısından oldukça önemlidir. Bu çalışmada kronik obstrüktif akciğer hastalarında, polisomnografi ile obstrüktif uyku apne sendromu OUAS sıklığını araştırmayı amaçladık. Yöntem: Çalışmaya kronik obstrüktif akciğer hastalığı KOAH tanısı olan 42 hasta 21 bayan, 21 erkek dahil edildi. Hastaların sigara anamnezi, yapılan kan gazı analiz KGA sonuçları, hematokrit değerleri ve antropometrik özelikleri kaydedildi. Gündüz aşırı uyku halini objektif olarak değerlendirmek amacıyla Epworth Uykululuk Skalası kullanıldı. Hastaların hepsine polisomnografi yapıldı. Bulgular: KOAH’lı hastalarda OUAS oranı %61.9 olarak bulundu. OUAS’lı olguların %30.7 hafif ,% 42.30 orta, %26.9 ağır OUAS sınıfındaydı. OUAS olan ve olmayan gruplar arasında demografik özellikler, solunum fonksiyon testi SFT , KGA ve hematokrit değerleri açısından istatistiksel olarak farklılık yoktu. Olguların %69’unda horlama, %61.9’unda gündüz aşırı uyku hali, %59.5’inde tanıklı apne saptandı. Horlama, tanıklı apne ve gündüz uyuklama şikayetleri olanlarda ayrı ayrı olmayanlara göre OUAS görülme sıklığı anlamlı olarak yüksekti. p
