Arthroscopic ACL reconstruction with patellar tendon

Abstract

Only recently has the majority of orthopedic surgeons recognized that anterior cruciate Iigament (ACL) insufficiency is one of the most common causes of knee instability and the most common cause due to knee Iigament insufficiency. Numerous procedures have been proposed and utilized to compensate for ACL insufficiency, although only a few have survived the test of time. Currently, arthroscopy-assisted patellar tendon substitutio for ACL insufficiency is utilized at our clinic. This technique essentially eliminates the necessary but substantial surgical trauma to the extensor mechanism and capsular structures of the knee joint that requires arthrotomy in delineating and developing the femoral and tibial tunnels. The arthroscopic technique significantly diminishes the intra and extraarticular adhesions that produce prolonged rehabilitation time in gaining flexion and extension. The results of arthroscopic ACL reconstruction with patellar-tendon reveaf that the static results are as good as those produced by arthrotomy, with a much fower morbidity rate.

Patellar tendon ile artroskopik ön çapraz bağ tamiri

Öz

Günümüzde ortopedistlerin büyük bir çoğunluğu ön çapraz bağ yetmezliğinin dizin instabilitesinde en sık rastlanan etkenlerden biri olduğunda birleşmektedir. Ön çapraz bağların rekonstrüksiyonunu sağlamak için çok değişik yöntemler denenmiş olmakla beraber en çok bir kaçı uzun süreli takipler sonunda popülaritesini koruyabilmiştir. Son üç yıldır ön çapraz bağ tamirinde greft olarak patellar tendon orta 1/3 bölümünü kullanıyoruz. Bugün artan deneyim ve artroskopi bilgimizle ön çapraz bağ tamirini artroskopik yardımlı olarak gerçekleştirmeye başladık. Artroskopik yardımlı teknikle femoral ve tibial tünelleri açmak için yapılan artrotomi ve bunun kapsüler yapı ve extensör mekanizma üzerine olumsuz travmalan da elimine edilmiştir. Artroskopik teknikle intra ve extra artiküler yapışıklık minimale indirildiği gibi, rehabilitasyon süresi kısalarak erken aktif yaşama dönüş sağlanmaktadır.
