A new technique for fixation of the foot with the Ilizarov fixator

Öz

Circular external fixators which are very often used in Orthopaedic and Traumatologic practice, sometimes ankle and foot may need to be included in fixator. Kirschner wires (K wire) are generally used by Russian and some western researchers for this purpose. But this procedure generates some problems for both patients and Surgeons. Major problems are easy contamination of wire, difficulty to wear shoes, difficulty to obtain clear radiological images with wires. We have been using Schanz pin instead of K wire in cases that foot should be included in fixation, since 1994. In this method, the number of pin on the skin diminishes, the risk of contamination reduces and patient can wear slipper. Moreover fixator is lighter and cheaper, to obtain clear radiological images is easier because less material is used for fixation of Schanz pins.

Öz

Ortopedi ve Travmatolojide çok sık kullanılan sirküler eksternal fiksatörlerde ayakbileği ve ayağı fiksatöre dahil etmek gerekebilmektedir. Ayağın fiksatöre dahil edildiği durumlarda Rus ve batılı müellifler tara-fından yaygın olarak Kirschner (K) telleri kullanılmaktadır. Ayağın yere yakın olması nedeni ile tel diplerinin kontaminasyonunun kolaylığı, hastanın ayakkabı giymesinin zorluğu, K tellerini tutan halkanın radyolojik görüntülemeyi güçleştirmesi hasta ve hekim açısından çeşitli sorunlara yol açmaktadır. Kliniğimizde 1994 yılından beri sirküler fiksatöre ayağı dahil ettiğimiz durumlarda K teli yerine 4 mm'lik Schanz vidası kullanmaktayız. Böylece ciltteki tel dibi sayısı azalmakta, pinler ayak dorsalinden kemiği tespit ettiği için kontaminasyon riski azalmakta, hasta daha rahat terlik giyebilmektedir. Ayrıca Schanz vidalarının tespiti için daha az materyal kullanılmakta, fiksatör daha hafif ve ucuz olmakta, radyolojik tetkik daha koyay yapılabilmekte-dir.
