Karşı Meme İnvaziv Lobüler Karsinomu: Önceki İnvaziv Duktal Karsinom Öyküsü Olan Bir Hastada MRG ile Saptanan ve Elastografi ile Ayırt Edilen İntramamarian Lenf Nodu Benzeri Lezyon

Abstract

Bu olgu sunumunda, invaziv duktal karsinom (IDC) tedavisinden üç yıl sonra karşı memede invaziv lobüler karsinom (ILC) tanısı alan nadir bir olgu sunulmaktadır. Tarama amaçlı yapılan meme manyetik rezonans görüntülemesinde (MRG) tesadüfen saptanan yeni lezyon, iyi sınırlı görünümü ve hilus benzeri ekojenitesi nedeniyle MRG ve ultrasonografide intramamari lenf nodunu taklit etmiştir. Shear-wave elastografi incelemesinde yüksek sertlik değerleri saptanmış ve malignite şüphesi doğmuştur. Kor biyopsi sonucunda invaziv lobüler karsinom tanısı doğrulanmıştır. Bu olgu, klinik olarak belirti vermeyen kanserlerin saptanmasında MRG’nin önemini ve benign yapıları taklit eden malign lezyonların ayırıcı tanısında elastografinin potansiyel rolünü vurgulamaktadır.

Contralateral Invasive Lobular Carcinoma Mimicking an Intramammary Lymph Node: A Case Report of MRI Detection and Elastography Differentiation in a Patient with Prior IDC

Abstract

We report a rare case of contralateral invasive lobular carcinoma (ILC) diagnosed three years after initial treatment for invasive ductal carcinoma (IDC). The new lesion, detected incidentally on screening MRI, mimicked an intramammary lymph node on MRI and ultrasound due to its well-circumscribed appearance and hilar-like echogenicity. Shear-wave elastography revealed high stiffness values, raising suspicion for malignancy. Core biopsy confirmed ILC. This case underscores the critical role of MRI in detecting clinically occult cancers and highlights elastography’s potential to unmask malignancies mimicking benign structures.
