Alt Ekstremiteye İlişkin Parametrelerin Mesane ve Bağırsak Fonksiyonları Üzerine Etkisi: Duchenne Musküler Distrofili Çocuklarda Bir İnceleme

Abstract

Öz Giriş ve amaç: Duchenne Musküler Distrofili (DMD) bireylerde mesane ve bağırsak fonksiyonları da etkilenmektedir. Çalışmamızda 5-12 yaş aralığında yürüme becerisi olan DMD’li çocuklarda alt ekstremite kas kuvvetinin, eklem hareket açıklığının (EHA), esneklik ve fonksiyonellik seviyesinin mesane ve bağırsak problemleri üzerindeki etkisini sağlıklı akranlarıyla karşılaştırarak incelenmesi amaçlandı. Gereç ve yöntemler: Çalışmaya, Grup 1: Sağlıklı çocuklar (n=12) ve Grup 2: DMD’li çocuklar (n=12) olmak üzere toplam 24 çocuk dahil edildi. Alt ekstremite kas kuvveti hand-held dinamometre cihazı ile, EHA gonyometre ile, esneklik otur-uzan testi ile, fonksiyonellik seviyesi “Çocuklar için Fonksiyonel Bağımsızlık Ölçümü (WeeFIM)” ile, mesane ve bağırsak fonksiyonları ise olan “Çocukluk Çağı Mesane ve Bağırsak Disfonksiyon Anketi (ÇÇMBDA)” ile değerlendirildi. Verilerin analizi basit doğrusal regresyon analizi ile gerçekleştirildi. Bulgular: Grup 1’de alt ektremite kas kuvvetlerinin ÇÇMBDA’ya ait toplam değer üzerinde negatif yönlü bir etkisi bulunurken (p<0,05), Grup 2’de ise herhangi bir alt ekstremite parametresinin anlamlı bir etkisi bulunmadı (p>0,05). Grup 1’de alt ekstremiteye ait fonksiyonellik parametrelerinin ÇÇMBDA’nın mesaneye ait değeri üzerinde anlamlı bir etkisi yok iken (p>0,05), Grup 2’de diz ekstansör kas kuvvetinin ve WeeFIM-kendine bakım alt parametresinin negatif yönlü anlamlı bir etkisi bulundu (p<0,05). Grup 1’de alt ekstremite EHA değerlerinin ÇÇMBDA’nın bağırsağa ait değeri üzerinde pozititf yönlü anlamlı bir etkisi bulunurken (p<0,05), Grup 2’de ise alt ekstremite kas kuvveti ve EHA değerlerinin pozititf yönlü anlamlı bir etkisi bulunurken (p<0,05). Sonuç: Bulgular, DMD’li çocuklarda alt ekstremite kas kuvveti ve kendine bakım düzeyi arttıkça mesane fonksiyonlarının tek başına iyileştiğini ve alt ekstremite kas kuvveti arttıkça bağırsak fonksiyonlarının ise tek başına kötüleştiğini göstermektektedir. Effects of Lower Extremity Parameters on Bladder and Bowel Functions: A Study in Children with Duchenne Muscular Dystrophy

Abstract

Abstract Introduction and aim: Duchenne muscular dystrophy (DMD) significantly affects bladder and bowel functions in affected individuals. This study aimed to examine the impact of lower extremity muscle strength, joint range of motion (ROM), flexibility, and functionality on bladder and bowel problems in children with DMD (aged 5-12 years), comparing them to healthy peers. Materials and methods: A total of 24 children were included: Group 1 (healthy children, n=12) and Group 2 (children with DMD, n=12). Lower extremity muscle strength was assessed using a hand-held dynamometer, ROM with a goniometer, flexibility with the sit-and-reach test, functionality with the " Functional Independence Measure for Children (WeeFIM)" scale, and bladder and bowel function with the "Childhood Bladder and Bowel Dysfunction Questionnaire (CBBDQ)." Data were analyzed using simple linear regression. Results: In Group 1, lower extremity muscle strength negatively affected the total CBBDQ score (p<0.05), while in Group 2, no significant effect of any lower extremity parameter was found (p>0.05). In Group 1, lower extremity functionality did not significantly affect the bladder score of the CBBDQ (p>0.05), whereas in Group 2, knee extensor strength and the WeeFIM self-care subscale negatively impacted bladder function (p<0.05). In Group 1, lower extremity ROM positively affected the bowel score of the CBBDQ (p<0.05), while in Group 2, muscle strength and ROM had a positive significant effect on bowel function (p<0.05). Conclusion: The findings suggest that as lower extremity muscle strength and self-care levels increase in children with DMD, bladder function improves independently, while bowel function deteriorates as muscle strength increases.
