Yumuşak Doku Sarkomlarının Yeniden Değerlendirilmesi: Sótero del Río Hastanesi'nde Görev Yapan Baş-Boyun Cerrahisi Ekibinin On Yıllık Deneyiminin Analizi

Abstract

Bu Editöryel, Sótero del Río Hastanesi'ndeki on yıllık cerrahi deneyimden yararlanarak yumuşak doku sarkomlarının yeniden değerlendirilmesiyle ilgilidir. Şili'de türünün ilk örneği olan bu retrospektif çalışma, bu nadir hastalığın anlaşılmasına büyük katkı sağlamaktadır. Bu haliyle, farklılaşmamış pleomorfik sarkomun baskınlığını ve sıklıkla ekstremitelerde yerleştiğini doğrulamaktadır; bu bulgular genel uluslararası literatürle de örtüşmektedir. Bununla birlikte, tasarımın retrospektif yapısı ve örneklemin mütevazı boyutu, bunun evrensel genelleştirilmesini kısıtlamaktadır. Önemli bir bulgu teyit edilmiştir: negatif cerrahi sınırlar elde etmek, lokal nüksün ve hasta sağkalımının iyileşmesinin tek bağımsız öngörücüsü olarak durmaktadır. Bundan böyle, ameliyat öncesi pozitron emisyon tomografisi ve spesifik adjuvan tedaviler gibi gelişmelere sınırlı erişim esasen dikkate alınmalıdır. Bu nadir ve karmaşık patolojinin yönetimi için alanında uzmanlaşmış multidisipliner ekiplerin çalışmaları ve tanı ve tedaviye erişimin iyileştirilmesi gerekmektedir.

Reevaluation of the Sarcomas of the Soft Tissues: An Analysis of the Decennial Experience of the Team of Head and Neck Surgery Residing at the Sótero del Río Hospital

Abstract

This missive doth concern the re-evaluation of soft tissue sarcomas, drawing upon the decade-long surgical experience at Sótero del Río Hospital. This retrospective study, being the first of its kind in the Chilean nation, doth stand as a contribution of high merit unto the understanding of this rare malady. As such, it confirms the predominance of undifferentiated pleomorphic sarcoma and the frequent location in the extremities, findings that accord well with the general international literature. Nevertheless, the retrospective nature of the design and the modest size of the sample doth restrict universal generalization thereof. Of note, a crucial discovery is affirmed: achieving negative surgical margins doth stand as the sole independent predictor of local recurrence and improved patient survival. Henceforth, the limited access to such advancements as preoperative positron emission tomography and specific adjuvant therapies must essentially be considered. Herewith, the establishment of specialized multidisciplinary teams and improved access to diagnostics and treatment are requirements from now onward in order of managing this rare and complex pathology.
