nfektif Endokardite Bağlı Mekanik Mitral Kapak Dehisensi ve Faktör II, VII, IX, X (Cofakt ®) Etkisi: Olgu Sunumu
Abstract
In this paper we aimed to present our 33-year-old male case that underwent operation due to prosthetic mitral valve dehiscence secondary to infective endocarditis developed 7 months after its implantation. Generally, cases with prosthetic mitral valve dehiscence due to infective endocarditis should be electively operated whenever they could tolerate surgery hemodynamically and in terms of laboratory parameters except for emergency indications. We think that the success of surgery would increase by stabilizing the hemodynamic and laboratory parameters promptly. For this purpose Cofact® (prothrombin complex concentrate) may be used to reverse extreme INR values when this cannot be achieved by fresh frozen plasma.

Prosthetic Mitral Valve Dehiscence Related to Infective Endocarditis and Effect of Factor II, VII, IX, X (Cofakt ®): Case Report
Abstract
Bu yazıda mekanik mitral kapak replasmanı sonrası 7. ayda gelişen infektif endokardite bağlı kapak dehisensi nedeniyle opere ettiğimiz 33 yaşında bir erkek hastayı sunulmuştur.İnfektif endokardite bağlı mekanik mitral kapak dehisensi olan hastaların, acil endikasyonlar dışında ameliyatı tolere edebilecek hemodinamik ve labaratuvar özelliklere sahip olduğu zaman ameliyata alınması gerekmektedir. Bu hemodinamik ve labaratuvar parametrelerinin hızlı bir şekilde stabilize edilmesi ile ameliyat başarısının artacağı kanaatindeyiz. Bu amaçla, taze donmuş plazma tedavisi ile INR yüksekliği gerilemeyen hastalarda Cofact® (protrombin kompleks konsantresi) kullanılabilir.
