An Unusual Case: The Migration of a Bullet into the Bladder and Covered with Stone

Abstract

    Intravesical foreign objects are rare but significant urological problem as they cause recurrent urinary tract infections, bladder stone formation and chronic irritation leading to malignant transformation. Although self-urethral infliction and iatrogenic etiologies are common, trauma-related etiologies and migration from an adjacent organ are also reported within literature. Here we present a case with a prediagnosis of bladder stone and an unexpected intraoperative diagnosis of intravesical bullet covered by stone identified during cystolithotripsy. Further investigation revealed a story of gunshot injury of her lower abdomen and a bullet left in the iliac bones thirty years before. Thus, the bullet was presumed to migrate from the pelvic bones to the urinary bladder.

Nadir Görülen Bir Olgu: Mesane İçinde Taşlaşmış Geç Kurşun Migrasyonu

Abstract

    Mesane içi yabancı cisimler nadir olmakla birlikte tekrarlayan idrar yolu enfeksiyonlarına, mesane taşı oluşumuna ve malign transformasyona yol açan kronik irritasyona neden oldukları için önemli bir ürolojik problemdir. Kendi kendine, üriner enfeksiyon ve iatrojenik etiyolojiler yaygın olmakla birlikte, literatürde travmaya bağlı etiyolojiler ve komşu organdan migrasyon da diğer etiyolojik sebepler olarak bildirilmiştir. Burada; mesane taşı ön tanısı konulup, beklenmedik şekilde sistolitotripsi sırasında üzeri taşla kaplı intravezikal mermi tanısı konulan çok nadir görülen bir olgu sunulmuştur. Hastanın öyküsünde, 30 yıl önce alt batına yönelik ateşli silah yaralanması ve sonrasında iliak kemiklerinde kalan bir mermi detayı mevcuttur. Böylece merminin pelvik kemiklerden idrar torbasına migrate olduğu varsayılmıştır.
