Color Doppler-Guided Superthin Zygomaticofacial Artery Perforator Flap for Reconstruction of a Lateral Orbital Wall Defect

Abstract

Lateral orbital wall defects are challenging to reconstruct because of the complex anatomy of the region and the need for simultaneous functional and aesthetic restoration. In selected cases, particularly after oncologic resection or tumor recurrence, local reconstructive options or skin grafts may be insufficient, and well-vascularized perforator flaps may be required. We report a 54-year-old patient with recurrent squamous cell carcinoma of the right lateral periorbital region who underwent wide local excision with clear oncologic margins. Preoperative color Doppler ultrasonography was used for perforator mapping and identified a reliable perforator within the zygomaticofacial vascular territory. A superthin zygomaticofacial artery perforator flap was planned and elevated in the superficial fascial plane. The perforator pedicle was meticulously dissected under magnification, and approximately 1.5 cm of pedicle length was obtained. The flap was rotated and inset without tension, achieving complete defect coverage. The postoperative course was uneventful, with no early or late complications. The flap demonstrated stable viability, satisfactory contour restoration, and minimal donor-site morbidity. The Doppler-guided superthin zygomaticofacial artery perforator flap may be a reliable reconstructive option for selected lateral orbital wall defects by enabling precise perforator localization, superficial fascial plane dissection, and transfer of thin, pliable, well-vascularized tissue with favorable aesthetic outcomes. Further studies with larger patient series are required to validate its reproducibility and long-term reliability.
