Overview of Code Blue Application in a University Hospital: How Realistic Are We, What Should We Do?

Abstract

Aim: The aim of this study is that drawing attention to importance of the code blue application,it’s effectiveness and reliabilit and sharing the malfunctions and problems in the operation and solution suggestions regarding them. Material and Methods: Code Blue Notification Forms between 2018-2022 were reviewed retrospectively. Age, gender, the unit where the call was given, the time and time period of the call, the time of arrival of the team, possible causes and the results of the application were evaluated. Results: 86 (44.8%) of the 192 code blue calls (CCT) registered were female, 106 (55.2%) were male, and the mean age was 53.23. Considering the rates of intervention places, it was seen that the hospital corridors and waiting rooms had the highest rate. There was no statistically difference in calls made before noon and in the afternoon. It was observed that 70% of the teams reached the call point in less than 1 minute. Considering the preliminary diagnoses made at the scene, syncope took the first place. Considering the cases of calls that require advanced life support; It was seen that 51 calls required resuscitation, while other calls were manageable with simple medical intervention. Considering the relationship between the actuality of MCS and the need for resuscitation in the intervention areas, it was observed that the probability of the calls in the processing units to be blue code was significantly higher (p<0.001), and the calls from the corridors and waiting rooms were mostly not blue codes. Conclusion: As a result of the study we conducted by examining the MKBF registered in our hospital, it was seen that the hospital blue code team and MKÇ system were created, but many unrealistic calls were made and the records were insufficient.

Bir Üniversite Hastanesinde Mavi Kod Uygulamasına Bakış: Ne Kadar Gerçekçiyiz, Neler Yapmalıyız?

Abstract

Amaç: Bu çalışmanın amacı; mavi kod uygulamasının önemi, etkinlik ve güvenilirliğine dikkat çekmek, işleyişteki aksaklıklar, sorunlar ve bunlarla ilgili çözüm önerilerini paylaşmaktır. Gereç ve Yöntemler: 2018-2022 yıllarında Mavi Kod Bildirim Formları (MKBF) retrospektif olarak incelendi. Yaş, cinsiyet, çağrının verildiği birim, çağrının verildiği saat ve zaman dilimi, ekibin ulaşma zamanı, olası nedenler ve uygulama sonuçları değerlendirildi Bulgular: Kayıtlara geçen 192 mavi kod çağrısının (MKÇ) 86’sı (%44,8) kadın, 106’sı (%55,2) erkek, yaş ortalaması 53,23 olarak saptandı. Müdahale yeri oranlarına bakıldığında en büyük oranın hastane koridorları ve bekleme salonları olduğu görüldü. Öğleden önce ve öğleden sonra olan çağrılarda sayısal açıdan istatistiksel olarak anlamlı bir fark görülmedi. Ekiplerin %70 oranında 1 dakikadan kısa sürede çağrı noktasına ulaştığı gözlendi. Olay yerinde konan ön tanılara bakıldığında senkop ilk sırada yer aldı. Çağrıların ileri yaşam desteği gerektiren durumlarına bakıldığında; 51 çağrının resüsitasyon gerektirdiği, diğer çağrıların basit tıbbi müdahale ile yönetilebilir olduğu görüldü. Müdahale yerlerinde MKÇ’nin gerçek olması ve resüsitasyon ihtiyacı ilişkisine bakıldığında işlem ünitelerindeki çağrıların mavi kod olma olasılığının belirgin yüksek olduğu (p<0,001), koridor ve bekleme salonlarından gelen çağrıların büyük oranda mavi kod olmadığı gözlendi. Sonuç: Yaptığımız çalışma sonucunda hastane mavi kod ekibi ve MKÇ sisteminin oluşturulduğu, ancak gerçek olmayan çok sayıda çağrı yapıldığı ve kayıtların yetersiz olduğu görülmüştür.
