Özet







Glomus timpanikum, promontoryum üzerindeki mukozada bulunan timpanik pleksusun kemoreseptör merkezleri olan Jacobson (IX) sinirinin timpanik segmenti veya Arnold (X) sinirinin aurikuler dalından kaynaklanabilir. Glomus timpanikum tümörü olan hastalarda genellikle işitme kaybı ve pulsatil tinnitus vardır. Bu tümör orta kulakta yer aldığından dolayı tanısında genellikle ilk planda odyolojik ve radyolojik tetkikler kullanılmaktadır. Tedavi olarak cerrahi yöntemler ve radyoterapi uygulanabilir. Bu yazıda, sol kulağında dolgunluk hissi ile kliniğimize başvuran otoskopik muayenesinde tam promontorium üzerinde çok küçük alanda kırmızı refle ve orta kulakta sıvı izlenimi veren, odyolojik ve konvansiyonel radyolojik tetkikleri tamamen normal olan ve parasentez yapılarak alınan biopsi sonucu paraganglioma tanısı konulan bir olgu sunuldu.

Abstract


Early stage glomus tympanicum







Glomus tympanicum may originate from auricular branch of Arnold (X) nerve or from tympanic segment of Jacobson's nerve (IX), which is the center of chemoreceptor of tympanic plexus found on the mucosa of promontorium. The patients with glomus tympanicum generally complain from hearing loss and tinnitus. As this tumor originates from middle ear, audiologic and radiologic methods are the prior methods of diagnosis. The preferable therapeutic approaches are surgery and radiotherapy. In this negoit, a patient suffering from aural fullness, examination findings of whom were red reflection of tympanic membrane on promontorium and fluid level of middle ear. Audiologic and conventional radiographic examinations were normal, and the paraganglioma diagnosis was confirmed by myringotomy and biopsy.
