ENDODONTİK ENFEKSİYONLARIN TEDAVİSİ İÇİN ANTİBİYOTİK REÇETE EDİLMESİ: TÜRKİYE'DE DİŞ HEKİMLERİ VE DİŞ HEKİMLİĞİ ÖĞRENCİLERİ ARASINDAKİ ANKET ARAŞTIRMASI

Öz

Amaç: Diş hekimliği öğrencilerinin ve diş hekimlerinin endodontik enfeksiyonlar için antibiyotik reçeteleme konusundaki görüşlerini değerlendirmek. Gereç ve Yöntemler: Çalışmamıza Necmettin Erbakan Üniversitesi Diş Hekimliği Fakültesi dördüncü/beşinci sınıf öğrencileri ve mezun olmuş diş hekimleri Google Formlar ile katılmıştır. Çalışmaya 139 diş hekimi (G1), 74 beşinci sınıf (G2) ve 63 dördüncü sınıf (G3) öğrencileri katılmıştır. Anket, 26 niteliksel ve niceliksel sorudan oluşmuştur. İlk 13 soruda, mezuniyet yılı, mezun olduğu diş hekimliği fakültesi, her iş günü başına görülen hasta sayısı, endodontik acil hasta sayısı ve endodontik tedaviler için mevcut antibiyotik reçete yazma kılavuzları bilinci gibi genel bilgiler kaydedildi. Sonraki 9 soruda, katılımcılardan antibiyotik yazıp yazmadıklarını söylemelerinin istendiği bir dizi varsayımsal klinik senaryo içermektedir. Geri kalan 3 soruda hangi antibiyotiği tercih ettikleri dozları ve süresi hakkında sorular bulunmaktadır. Veriler, tanımlayıcı istatistikler ve anlamlılık düzeyi p=0.05 ki-kare testi kullanılarak analiz edilmiştir. Bulgular: Anket sorularının iletildiği 463 kişiden 276 kişi cevaplamıştır ve oranı %59.6’dır. Öğrencilerin mezun diş hekimlerinden istatistiksel olarak anlamlı derecede daha fazla rehber kılavuzların farkında olduğunu belirtmiştir (p=0.008). Akut apikal periodontitisli, sistemik tutulumu olmayan-lokalize fluktuan şişliği olan akut apikal apseli ya da uygun kök kanal tedavileri ile çözülemeyen kronik eksüdasyon varlığında diş hekimleri öğrencilere göre istatistiksel olarak anlamlı daha fazla antibiyotik tercih etmişlerdir (p<0.05). Sonuç: Diş hekimlerin klinik deneyim süresi arttıkça endodontik rehber kılavuzlara göre daha fazla antibiyotik tercih etmişlerdir. Eğitimin devamlılığı ve bilginin sürekli yenilenmesi ile endodontik acil tedavilerde gereksiz antibiyotik reçete edilmesi önlenebilir.

Antibiotic Prescribing for the Treatment of Endodontic Infections: a Survey of Dentists and Dental Students in Turkey

Öz

Background: To evaluate the views of dental students and dental practitioners on antibiotic prescribing for endodontic infections. Methods: Necmettin Erbakan University Faculty of Dentistry fourth/fifth-years students and graduated dentists participated in our study with Google Forms. 139 dentists(G1), 74 fifth-years(G2), and 63 fourth-years(G3) students participated in the study. The survey consisted of 26 qualitative and quantitative questions. In the first 13 questions, general information such as the year of graduation, graduated faculty, the number of patients per working day, the number of endodontic emergency patients, and the awareness of current antibiotic prescription guidelines for endodontic treatments were recorded. The next 9 questions consisted of a series of hypothetical clinical scenarios where participants were asked to tell if they were writing antibiotics. In the remaining 3 questions were about which antibiotic they prefer, their dosage, and duration. Data were analyzed using descriptive statistics and chi-square test with a significance level p=0.05. Results: 276 of 463 people answered the questionnaire and the rate is 59.6%. It was stated that the students were statistically significantly more awareness of guidelines than the graduate dentists (p=0.008). In the presence of acute apical periodontitis, acute apical abscess without systemic involvement-localized fluctuant swelling, or chronic exudation that cannot be resolved with appropriate root canal treatments, dentists preferred more antibiotics than students (p<0.05). Conclusion: As the period of clinical experience of dentists increased, they preferred more antibiotics than endodontic guides. Unnecessary antibiotic prescription can be prevented in endodontic emergency treatments by continuing education and renewing information.
