Le Fort I Osteotomisinin Fiksasyonu Açısından İskeletsel Sınıf II ve Sınıf III Hastalarda Anterior Maksiller Bölgenin Üç Boyutlu Değerlendirilmesi

Abstract

Amaç: Bu çalışmanın amacı, iskeletsel Sınıf II ve Sınıf III hastalarda anterior maksiller bölgenin hacim ve yoğunluğunu konik ışınlı bilgisayarlı tomografi (KIBT) kullanarak değerlendirmek ve bu iki hasta grubu arasında plak ve vidaların stabilizasyonunu etkileyebilecek farkların olup olmadığını araştırmaktır.

Gereç ve Yöntemler: 48 hastanın (24 Sınıf II ve 24 Sınıf III) KIBT görüntüleri retrospektif olarak incelendi. Maksiller anterior bölgenin (orta hat ile kanin dişler arası) hacmi, yüzey alanı ve ortalama yoğunluğu tomografi görüntülerinden hesaplandı. Ayrıca, maksiller kanin, lateral ve santral dişlerin nazal fossaya olan mesafeleri ölçüldü.

Bulgular: İskeletsel Sınıf II ve Sınıf III hastalar arasında maksiller sağ ve sol taraflardaki hacim, yüzey alanı, yoğunluk ile maksiller kanin, lateral ve santral dişlerin nazal fossa tabanına olan dikey yükseklik ölçümleri arasında istatistiksel olarak anlamlı bir fark bulunmadı (p>0.05). Cinsiyete göre sağ ve sol taraflardaki hacim ve yüzey alanı ölçümleri arasında ise istatistiksel olarak anlamlı bir fark tespit edildi (p<0.05).

Sonuçlar: İskeletsel Sınıf II ve Sınıf III hastalar arasında Le Fort I osteotomisi sırasında anterior bölgede kullanılan plak ve vidaların stabilizasyonunu etkileyebilecek morfolojik farklılıklar bulunmamaktadır. Ancak, anterior diş kökleri ile nazal fossa arasındaki mesafenin ameliyat öncesi değerlendirilmesi ve vidaların yerleştirilmesi sırasında bu mesafelerin dikkate alınması, diş köklerine zarar verilmesini önlemek ve ilgili dişlerin vitalitesini korumak açısından önemlidir.

Three-dimensional Evaluation of the Anterior Maxillary Region in Skeletal Class II and Class III Patients in Terms of Fixation of Le Fort I Osteotomy

Abstract

Background: This study aims to evaluate the volume and density of the anterior maxillary region in Class II and Class III patients using cone-beam computed tomography (CBCT) and to investigate whether there are differences between these two patient groups that could affect the stabilization of plates and screws.

Methods: CBCT images of 48 patients (24 Class II and 24 Class III) were retrospectively examined.The volume of the maxillary anterior region (between the midline and the canine teeth), surface area and average density were calculated from the tomography images. Additionally, the distances of the maxillary canine, lateral and central teeth to the nasal fossa were measured.

Results: No statistically significant differences were found between skeletal Class II and III patients in maxillary volume, surface area, density, or the vertical height of maxillary teeth to the nasal fossa floor (p>0.05). A statistically significant difference was observed between volume and surface area measurements on the right and left sides according to gender (p<0.05).

Conclusion: There are no morphological differences between class II and class III patients that may affect the stabilization of the plates and screws used in the anterior region in Le Fort I osteotomy. However, preoperative evaluation of the distance between the anterior tooth roots and the nasal fossa and taking these distances into account when placing the screws is important in order to prevent damage to the tooth roots and to preserve tooth vitality.
