İntravenöz Askorbik Asid İle Birlikte Oral Metilen Mavisi Kullanılan, Prilokaine Bağlı Methemoglobinemi: Vaka Takdimi

Abstract

Methemoglobinemi; kanda methemoglobin (iki değerli ferröz hemoglobin yerine üç değerli ferrik demir içeren türü) düzeyinin
normalden daha yüksek olmasına bağlı gelişen klinik tablodur. Methemoglobinemi konjenital/genetik ve edinsel nedenlere bağlı
gelişebilir. Konjenital methemoglobinemiler, yapısal hemoglobin bozuklukları ve methemoglobin redüksiyon enzimlerindeki eksikliklere
bağlı olarak gelişir. Edinsel methemoglobinemiler ise oksidan özelliğe sahip ilaç, kimyasal madde ve gıdalara maruziyet yanında süt
çocukluğu döneminde görülen bazı hastalıklara bağlı gelişebilir. Prilokain içeren lokal anestezik kullanılarak sünnet yapıldıktan sonra
solunum sıkıntısı ve siyanozu başlayan 3.5 aylık erkek bebek hastanemize yatırıldı. Kan methemoglobin düzeyi %31.5 bulundu ve
methemoglobinemi tanısı konuldu. İntravenöz askorbik asid ile birlikte oral metilen mavisi uygulanan hastanın 3. saatinde klinik
bulgularında düzelme izlendi ve 12. saatinde methemoglobin düzeyi normal olarak bulundu. Methemoglobinemi vakalarında intravenöz
askorbik asid ile birlikte oral metilen mavisi de tedavi seçenekleri arasında değerlendirilebilir.

Intravenous Ascorbic Acid and Oral Methylene Blue Treatment in Prilocaine-induced Methemoglobinemia: A Case Report

Abstract

Methemoglobinemia is a disorder characterized by the presence of a higher than normal level of methemoglobin (is a form of hemoglobin
that contains ferric [Fe3+] rather than ferrous [Fe2+] iron) in the blood. Methemoglobinemia can develop due to acquired and
congenital / genetic causes. Congenital methemoglobinemia develops due to structural deficiencies in hemoglobin and methemoglobin
reduction enzyme disorders. Acquired methemoglobinemia develops due to the exposure drugs, chemicals and foods having oxidant
property as well as some disorders observed during infancy. Three and half month-old boy was admitted with respiratory distress
and cyanosis after circumcision due to a a local anesthetic drug, prilocaine. Blood methemoglobin level was found as 31.5% and was
diagnosed with methemoglobinemia. Three hours after intravenous ascorbic acid and oral methylene blue were administered, clinical
symptoms were improved and methemoglobin level was found to be normal at 12th hours. In methemoglobinemia cases, treatment
options might include oral methylene blue combined with intravenous ascorbic acid.
