Submakuler Hemoraji Olgusunda İntravitreal Aflibersept

Abstract

Ellisekiz yaşında erkek olgu, sol gözde 1 aydır devam eden görme azalması şikayeti ile başvurdu. oftalmolojik muayenesinde görme keskinliği sağ gözde 0,8, sol gözde 0,2 idi. Göz içi basınçları (GİB) sırasıyla 17 ve 13 mmHg ölçüldü. Biyomikroskopide sağ göz evre 2 nükleer skleroz, sol göz psödofakikti. Fundus muayenesinde sağ doğal, sol makulada yaklaşık 8-9 optik disk çapında subretinal hemoraji, üst temporal arkuat hizasında hiperpigmente lezyon saptandı. Fundus Fluoresein anjiografide(FFA) sol gözde yaklaşık 8-9 optik disk çapında koroid blokajına neden olan geniş bir hipofloresan alan ve üst temporal arkuat hizasında geç dönemde belirginleşen hiperfloresan alan mevcuttu. Optik koherens tomografide (OKT) merkezi makula kalınlığı sağ gözde 243 µm, sol gözde 641 µm idi. Olgunun muayene bulguları ve tetkikler sonucunda submakular hemoraji tanısı konuldu. Olgumuza tedavi seçenekleri arasından intravitreal aflibercept enjeksiyonu uygulandı. Uygulanan dört doz intravitreal aflibersept sonrasında olgunun görme keskinliğinin 0,6’ya yükseldiği, fundus muayenesinde subretinal hemorajinin kaybolduğu ve merkezi makula kalınlığının 388 µm’ye gerilediği görüldü. Submakuler hemoraji olgularında intravitreal aflibercept enjeksiyonunun, primer tedavi olarak düşünülebileceğine dikkat çekmek istedik.
