Safra Yolu Cerrahisi Sonrası Görülen Bakteriel İnfeksiyonlar ve Kan Transfuzyonları Arasındaki İlişki

Abstract

Kan transfüzyonu çeşitli risk ve yan etkiler ile ilgilidir. Bu çalışmada amacımız safra yolu cerrahisi sonrası efektiF komplikasyonlar ile kan transfüzyonu arasında ilişki olup olmadığını araştırmaktadır. Bir genel cerrahi kliniğinde retrospektif olarak safra yolları hastalığı nedeniyle ameliyat edilen 163 hastada peroperatif olarak kan transfüzyonu yapılanlarda kan transfüzyonu ile infektif komplikasyon görülme sıklığını araştırdık. Kliniğimizde 49 hastada postoperatif enfeksiyon gelişmişti. Transfüzyon yapılmayan 154 hastanın 44’ünde (%28,5) enfeksiyöz komplikasyon gelişirken transfüzyon yapılan 9 hastanın 4’ünde (%36) bu durum belirlendi. Hastalarca alınan kan miktarı arttıkça enfeksiyöz komplikasyon oranı artmaktadır. Enfeksiyon gelişme oranının akciğer hastalığı, koledoğa cerrahi girişim ve T-tüp uygulaması ile arttığını belirledik. Safra yollarına cerrahi girişim yapılan hastalarda hastanede postoperatif enfeksiyon gelişmesinde kan transfüzyonunun istatistiki olarak anlamlı olmasa da bağımsız bir risk faktörü olduğunu belirledik.

The Relation of Blood Transfusion and Bacteriel Infection After Biliary Tract Surgery

Abstract

Blood transfusions are linked with certain risks and side effects. The aim of our study was to examine the association between blood transfusion and infectious complications after biliary operations. We retrospectively studied the relationship between perioperative blood transfusion and postoperative infectious complications in 163 consecutive patients undergoing surgery for biliary disease. Forty nine patients developed postoperative infections after biliary tract surgery in a General Surgery Department. Of the 154 patients who did not receive transfusion 44 (28 per cent) developed infectious complications compared with 4 (36 per cent) of the 9 patients who received blood. The number of units blood received by patients who developed infectious complications exceeded the number for patients with infectious complications. The development of infections was significantly associated pulmonary diseases, operation on the common bile duct, and T-tube drainage. We suggest that transfusion is one of the independent risk factor in the development of postoperative infection in hospital in patients who have had biliary operations, although it is not significant.
