Kurum Bakımındaki Otizm Spektrum Bozukluğu Olan Çocuklarda Uygulanan Koruyucu Ağız ve Diş Sağlığı Programının Kısa Dönem Sonuçları

Abstract

Amaç Otizm spektrum bozukluğu (OSB) olan çocuklar, anksiyete, duyusal hassasiyetler ve diş tedavileri sırasında sınırlı iş birliği nedeniyle ağız ve diş sağlığı hizmetlerine erişimde güçlük yaşayabilmektedir. Erken yaşta uygulanan koruyucu ağız ve diş sağlığı programları, ağız hijyeni alışkanlıklarını geliştirebilir ve diş tedavisine yönelik olumlu tutumların oluşmasına katkı sağlayabilir. Bu çalışmanın amacı, kurum bakımındaki OSB tanılı çocuklarda yapılandırılmış bir ağız ve diş sağlığı koruyucu programının ağız hijyeni durumu ve diş hekimi ile iş birliği düzeyi üzerindeki kısa dönem etkilerini değerlendirmektir. Gereç ve Yöntemler Etik kurul onayının alınmasının ardından Romanya'nın Mureș ilindeki sekiz kurumdan OSB tanılı 38 çocuk çalışmaya dâhil edildi. Müdahale programı; ağız ve diş sağlığı eğitimi, gözetimli koruyucu uygulamalar ve çocuklara, bakım verenlere ve ebeveynlere özel olarak hazırlanmış eğitim broşürlerinin dağıtılmasını içermektedir. Ağız muayeneleri başlangıçta ve iki aylık takip sonunda gerçekleştirildi. Diş çürüğü deneyimi Çürük, Eksik ve Dolgulu Diş (DMFT) indeksi ile, ağız hijyeni Silness-Löe plak indeksi ile değerlendirildi. Çocukların muayene sırasındaki iş birliği düzeyi dört basamaklı bir iş birliği ölçeği kullanılarak kaydedildi. İstatistiksel analiz Wilcoxon işaretli sıralar testi ile gerçekleştirildi ve anlamlılık düzeyi p<0,05 olarak kabul edildi. Bulgular Başlangıçta ortalama DMFT değeri 4,65 iken iki ay sonunda 4,73 olarak bulundu ve bu fark istatistiksel olarak anlamlı değildi (p>0,05). Ortalama plak indeksi 1,39'dan 1,15'e düşmesine rağmen bu azalma da istatistiksel olarak anlamlı değildi (p>0,05). Buna karşın, çocukların diş muayenesi sırasındaki iş birliği düzeyi uygulanan program sonrasında anlamlı düzeyde arttı (p<0,00001). Bakım verenler ve ebeveynler ağız hijyeni farkındalığı ile diş fırçalama alışkanlıklarında olumlu değişiklikler bildirdi ve katılımcı kurumların büyük çoğunluğunda ağız hijyeni uygulamalarında iyileşme gözlendi. Sonuç Bu kısa dönemli çalışmanın sınırlılıkları çerçevesinde, uygulanan koruyucu programın çocukların diş muayenesi sırasındaki iş birliğini anlamlı düzeyde artırdığı ve gelecekteki diş tedavilerini kolaylaştırabileceği gösterilmiştir. İki aylık takip süresinde DMFT ve plak indekslerinde istatistiksel olarak anlamlı değişiklik saptanmamış olsa da, elde edilen bulgular, çocukların alışık olduğu ortamda tekrarlanan koruyucu uygulamaların kurum bakımındaki OSB'li çocuklarda olumlu ağız ve diş sağlığı davranışlarını destekleyebileceğini düşündürmektedir. Kalıcı ağız ve diş sağlığı kazanımlarının sağlanabilmesi için bakım verenlerin aktif katılımını içeren uzun dönemli koruyucu programlara gereksinim vardır.

Short-Term Outcomes of a Dental Prevention Program in Institutionalized Children with Autism Spectrum Disorder

Abstract

Objectives Children with autism spectrum disorder (ASD) often experience difficulties in accessing dental care because of anxiety, sensory sensitivities, and limited cooperation during dental procedures. Preventive oral health programs introduced at an early age may improve oral hygiene practices and facilitate positive attitudes toward dental treatment. This study aimed to evaluate the short-term effects of a structured dental prevention program on oral hygiene status and cooperation during dental examinations among institutionalized children with ASD. Material and Methods Thirty-eight children diagnosed with ASD from 8 residential institutions in Mureș County, Romania, were enrolled after obtaining ethical approval. The intervention consisted of oral health education visits, supervised preventive activities, and the distribution of specially designed educational leaflets to children, caregivers, and parents. Oral examinations were performed at baseline and after a 2-month follow-up. Dental caries experience was assessed using the Decayed, Missing, and Filled Teeth (DMFT) index, oral hygiene was evaluated using the Silness-Löe plaque index, and the children's level of cooperation during examination was recorded using a 4-point cooperation scale. Statistical analysis was performed using the Wilcoxon signed-rank test with a significance level of P < 0.05. Results The mean DMFT score showed no significant change (4.65 vs. 4.73; P > 0.05). The mean plaque index decreased from 1.39 to 1.15, without statistical significance (P > 0.05). In contrast, cooperation during dental examinations improved significantly following the intervention (P < 0.001). Observational feedback from caregivers indicated improved oral hygiene engagement among participants. Conclusion Within the limitations of this short-term study, the preventive program significantly improved children's cooperation during dental examinations. Although no significant changes were observed in DMFT or plaque index, the findings suggest that structured preventive interventions delivered in familiar environments, with caregiver involvement, may enhance acceptance of dental care in children with ASD. Long-term studies are needed to assess sustained effects on oral health outcomes.
