Spontan Vetriküler Kanamaya Bağlı Oluşan Hidrosefalide Kullanılan Drenaj Kit Türünün Mortalite Üzerindeki Etkileri

Abstract

Amaç: Çalışmamızın amacı ventriküler kanama sonucu
gelişen hidrosefalide kullanılan klasik şant ile monitorlu
şantların mortalite üzerindeki etkilerini incelemektir.
Yöntem: Nisan 2011-Ağustos 2014 tarihleri arasında
birden fazla merkezde takip edilen 60 (35’i bayan, 25’i
erkek) vetriküler kanamaya bağlı hidrosefali vakası
retrospektif olarak analiz edildi. Hastalar iki gruba ayrıldı.
Grup 1’de ki 30 hastaya klasik vetriküler drenaj seti
takıldı. Grup 2’de ki 30 hastaya ise mönitorlu ventriküler
drenaj seti takıldı. Hastalar rastgele seçildi ve tüm
hastaların geliş Glaskow Koma Skorları kaydedildi.
Bulgular: Birinci grup da Glaskow Koma Skoru 6’nın
altında olan hastalardan 10 hasta, Glaskow Koma Skoru
6’nın üzerinde olan 5 hasta eksitus oldu. İkinci grup da ise
Glaskow Koma Skoru 6’nın altında olan 7 hasta, Glaskow
Koma Skoru 6’nın üzerinde olan 3 hasta eksitus oldu.
Birinci grup da 4 ve ikinci grup 2 hastaya da kalıcı
ventriküloperitoneal şant takıldı.
Sonuç: Kullanılan şant sistemleri arasında maliyet farkının
fazla olmasına rağmen, her iki grup arasında mortalite
açısından anlamlı fark bir tespit edilmemesine rağmen ve
mortalite üzerine ilk geliş Glaskow Koma Skorunun daha
anlamlı olduğu tespit edilmiştir.
