Retrospective Analysis of Extracranial Schwannoma on Head and Neck Region And Surgical Treatment

Abstract

Aim: Schwannomas originating from Schwann cells are quite rare and benign
tumors of the peripheral nerve sheath. The aim of the present study is to
analyze the surgical and clinical outcomes of patients with extracranial head
and neck schwannomas.

Material and Method: The patients diagnosed with extracranial
schwannoma localized at the head and neck region were included in this
retrospective study.Demographic characteristics, clinical symptoms, nerves
originating from schwannomas and surgical outcomes were analyzed
retrospectively.

Results: The age of the patients varied between 16 to 57 years.Of the 14 patients,
6 (42.8%) were female and 8 (57.2%) were male. Two (14.2%) patients had a
history of Von Recklinghausen disease (Type I neurofibromatosis). The affected
nerves were the cervical plexuses (n=3), brachial plexus (n=1), vagus nerve
(n=2), sympathetic chain (n=1) and lingual nerve (n=1). The nerve origin of the
schwannoma could not be detected in 6 cases. Tumour excision and/or enucleation
were performed in 14 patient and none of the patients except one had a
permanent neural deficit postoperatively. A patient with schwannoma originating
from brachial plexus showed motor weakness in the right arm postoperatively.

Conclusion: Schwannomas, which are usually incidentally detected, are benign
tumors of the peripheral nerves and sometimes may be associated with neurofibromatosis
types 1 and 2. The recommended treatment of extracranial schwannomas of the
head and neck region is tumour excision or enucleation. The nerve originating
from the schwannoma should be identified and the integrity of the nerve should
be maintained, especially if it is the motor nerve.

Baş ve Boyun Bölgesi Ekstrakraniyal Schwannomlarının Retrospektif Analizi ve Cerrahi Tedavisi

Abstract

Amaç: Schwann hücrelerinden kaynaklanan Schwannomlar, periferik sinir kılıfının
oldukça nadir ve iyi huylu tümörleridir. Bu çalışmanın amacı, ekstrakraniyal
baş ve boyun schwannoma hastalarının cerrahi ve klinik sonuçlarını analiz
etmektir.

Yöntem: Bu retrospektif çalışmaya baş ve boyun bölgesinde lokalize ekstrakranial
schwannoma tanısı konan hastalar dahil edildi. Demografik özellikler, klinik
semptomlar, schwannomlardan kaynaklanan sinirler ve cerrahi sonuçlar
retrospektif olarak incelendi.

Bulgular: Hastaların yaşı 16 ile 57 arasında değişmekteydi. 14 hastanın 6'sı (% 42,8)
kadın, 8'i (% 57,2) erkek idi.İki hastada (% 14.2) Von Recklinghausen hastalığı
öyküsü vardı (Tip I nörofibromatozis).Schwannomun kaynaklandığı sinirler
servikal pleksus (n = 3), brakiyal pleksus (n = 1), vagus siniri (n = 2),
sempatik zincir (n = 1) ve lingual sinirdi (n = 1). 6 olguda schwannomun köken
aldığı sinir tespit edilemedi.Tümör eksizyonu ve / veya enükleasyonu 14 hastaya
uygulandı ve bir hasta dışında hiçbir hastada ameliyat sonrası kalıcı bir motor
nöral bozukluk gelişmedi. Brakiyal pleksustan kaynaklanan schwannomalı bir
hastada ameliyat sonrası sağ kolda motor güçsüzlük gelişti.







Sonuç: Genellikle rastlantısal olarak tespit edilen schwannomlar periferik
sinirlerin iyi huylu tümörleridir ve bazen nörofibromatozis tip 1 ve 2 ile
birlikte görülebilirler. Baş ve boyun bölgesindeki ekstrakraniyal
schwannomaların önerilen tedavisi tümör eksizyonu veya
enükleasyonudur.Schwannomun köken aldığı sinir tanımlanmaya çalışılmalı ve
özellikle motor sinirse sinirin bütünlüğü korunmaya çalışılmalıdır.
