EVALUATION OF THE EFFECTS OF PROTHETIC REHABILITATION WITH MAXILARY OBTURATOR PROSTHESES ON PSYCHOLOGICAL IMPROVEMENT

Abstract

Objective: Obturator prostheses are the treatment of choice for maxillofacial prosthetic rehabilitation of intraoral defects. These treatments also affect the psychology of the patients depending on their moods in emotional situations. The aim of this clinical study was to determine the psychological reflections of treatment by identifying changes in 5 distinct emotional states before and 6 months after prosthetic treatment with a hollow balloon-type resection obturator prosthesis on metal substructure skeletal prosthesis in patients with Aramany class 1 maxillary defects. Materials and methods: 85 patients (N = 85) with intraoral deformity who accepted with informed consent form, applied a multiple response questionnaire used by Reabilitaçao de Bauru Hospital Psychology Department of Reabilitaçao de Bauru Hospital in 5 emotional state areas before and after prosthetic treatment (TS). [n1: personal image (QA), n2: sociality (S), n3: family relationships (AI), n4: professional competence (PY), n5: struggle with stress (S)] subjectively perceived mood symptoms (emotional) Psychological effects of the patients were evaluated in terms of problems, problems, disadvantages, difficulty, disorder, and symptoms such as emotional positivity and healing. Data were analyzed by using descriptive statistics and fisher’s exact test on the sum of the number of positive (O) or Disorder (B) symptom response numbers (n1, n2, n3, n4, n5) reported by the patients for their moods and the significance of the difference between these changes was examined (p. <0.01, p <0.05). Results: According to the n1, n2, n3, n4, n5 groups, the changes in mood symptoms that were perceived and described subjectively in 5 emotional state areas before and after treatment; According to the descriptive statistical evaluation of the mean data obtained by summing the number of patients (nort.);n Average. Disorder after treatment: 10.2; nort. Pre-treatment Disorder: less than 43 were found. N = 85; The mean number of patients who reported Post-Treatment Disorder among the patients decreased significantly, while the number of patient satisfaction increased (n mean. Post-Treatment Positive: 74.8). According to Fischer’s exact test data, changes in pre-treatment and post-treatment disorder and positivity mood symptoms of family relationships and stress emotional state area were significant (p <0.05). According to the evaluations in the parameters of the other 3 emotional state areas; Changes in mood before and after treatment were found to be statistically significant (p <0.01). Conclusion: Significant improvements in mood and psychology of patients receiving maxillary obturator therapy in areas reflecting emotional state such as body appearance, social interaction, family relationships, professional life and life motivation due to the ability to fight stress are provided.

MAKSİLLER OBTURATÖR PROTEZLERİ İLE PROTETİK REHABİLİTASYONUN PSİKOLOJİK İYİLEŞME ÜZERİNE ETKİLERİNİN DEĞERLENDİRİLMESİ

Abstract

Giriş: Baş-boyun tümörlerinin cerrahi tedavileri ve konjenital, herediter veya travmatik orofasiyal deformiteler nedeniyle hastalarda fonksiyonel ve estetik sorunların yanı sıra, psikolojik problemler de gözlenmektedir. İntraoral defektlerin maksillofasiyal protetik rehabilitasyonu için tedavi seçeneği obturator protezleridir. Cerrahi girişimler sonrasında yapılan bu protetik rehabilitasyonlar fonksiyonun restorasyonu ile birlikte, hastaların duygusal durumlarda hissettikleri ruh hallerine bağlı olarak psikolojisine de etki etmektedir. Amaç: Sunulan bu klinik çalışmanın amacı, Aramany sınıf 1 maksiller defektleri olan hastaların metal alt yapılı döküm iskelet protez üstü içi boş balon tipi bir rezeksiyon obturatör protezleri ile protetik tedavi öncesinde ve 6 ay sonrasında, belirli 5 ayrı duygusal durum alanı [1: kişisel görüntü (KG), 2: sosyallik (S), 3: aile ilişkileri (Aİ), 4: profesyonel yeterlilik (PY), 5: stres ile mücadele (S)] için öznel olarak algıladıkları (kendilerinde hissettikleri) ruh hali semptomları (duygusal sorun = problem = dezavantaj = zorluk = bozuk ya da duygusal olumlu değişiklik = olumluluk = iyileşme şeklinde) üzerindeki değişiklikleri belirleyerek tedavinin psikolojik yansımalarını ortaya çıkarmaktır. Sıfır hipotezimiz; maksiller rezeksiyon oturatör protezleri ile protetik rehabilitasyon tedavisi uygulanan maksillofasyal defektli hastaların, 5 ayrı duygusal durum alanlarında öznel olarak algıladıkları, kendilerinde hissettikleri ruh hali semptomları üzerinde herhangi bir olumlu değişiklik yapdığı yönündedir. Gereç ve yöntem: İntraoral deformitesi olan 85 hastaya (N=85) Sao Paulo Üniversitesi, Reabilitaçao de Bauru Hastanesi Psikoloji Bölümünün protez programı için kullandığı çoklu cevap anketi uygulanarak protetik tedavi öncesi (TÖ) ve sonrasında (TS) hastaların 5 duygusal durum alanındaki semptomları üzerinden hastalardaki psikolojik etkiler değerlendirildi. Veriler hastaların anketteki 5 duygusal durum alanında değerlendirdikleri ruh halleri için bildirdikleri olumluluk (O) ve/veya Bozukluk (B: zorluk, dezavantaj, olumsuzluk) semptomu cevap sayılarının toplamı (n1, n2, n3, n4, n5) üzerinden tanımlayıcı istatistikler ve Fisher’s exact test kullanılarak bu değişiklikler arasındaki farkın anlamlı ve önemli olup olmadığı incelendi (p<0.01, p<0.05). Bulgular: Hastaların tedavi öncesi ve sonrası 5 adet duygusal durum alanında gelişen ve öznel olarak algıladığı ve tariflediği ruh hali semptomlarındaki değişikliklerinin n1, n2, n3, n4, n5 gruplarına göre; hasta sayısının toplanması ile elde edilen ortalama verilerin (nort.) tanımlayıcı istatistiksel değerlendirmesine göre; nort.TSB: 10.2; nort.TÖB: 43’den daha düşük bulunmuştur. N=85’e göre; hastalar arasında TSB bildirenlerin değerlerinin ortalama sayısı, önemli bir ölçüde azalırken, hasta memnuniyetinin sayısında artış belirlenmiştir (nort.TSO: 74.8). Fischer’s exact test verilerine göre de, aile ilişkileri ve duygusal durum alanına ait TÖ ve TS bozukluk ve olumluluk ruh hali semptomlarındaki değişikliklerin anlamlı olduğu görülmüştür (p<0,05). Diğer 4 duygusal durum alanına ait parametrelerdeki değerlendirmelere göre de; TÖ ve TS ruh hallerindeki değişiklikler istatistiksel olarak anlamlı olduğu bulunmuştur (p<0,01). Sonuç: Çene-yüz cerrahisi geçiren ve maksiller obturatör tedavisi alan hastaların tedavi sonrasında vücut görünüşü, sosyal etkileşim, aile ilişkileri, profesyonel yaşam ve stres ile mücadele becerisine bağlı olarak gelişen yaşam motivasyonu gibi duygusal durumu yansıtan alanlarda hissettikleri ruh halleri ve dolayısıyla, psikolojilerinde belirgin ölçüde iyileşmeler sağlanmaktadır.
