Abstract

Retrospective Evaluation Of Non-Hospital Births In Turkish Citizens And Immigrant Populations In Istanbul: A Single-Center Experience

Abstract

Objective: To analyze the locations and reasons for out-of-hospital births admitted to our hospital, evaluate the feasibility of planned home birth in Istanbul, Türkiye, and
examine key aspects of follow-up care for newborns delivered outside hospital settings.
Methods: This retrospective study included 87 newborns delivered outside a hospital between March 2014 and September 2019 and subsequently admitted to our facility.
Birth location and reasons for out-of-hospital delivery were recorded. Pregnancies were classified as high- or low-risk according to the American College of Obstetricians
and Gynecologists criteria. Demographic, clinical, and laboratory data were analyzed. Admission diagnoses, length of hospital stay, and mortality rates were compared with
those of 2,779 newborns delivered in our hospital in 2019. Statistical analyses were performed using SPSS Statistics 22.0 (Armonk, NY: IBM Corp.).
Results: The majority of women who delivered outside the hospital (n=52, 59.7%) had not received prenatal care, and nearly half (n=42, 48.2%) were foreign nationals
from conflict-affected or developing countries. Among women who delivered at home, 13 (14.9%) did so voluntarily, of whom 11 (84.6%) had not received prenatal care.
Among unplanned out-of-hospital births, 56 (75.6%) occurred due to inability to reach a hospital in time. Additionally, 11 (14.8%) cases were attributed to neglect or lack
of education, and 4 (5.4%) to unwanted pregnancies. Overall, 32.1% of pregnancies were classified as high-risk and 67.9% as low-risk. Preterm birth occurred in 6.9% of
cases, while 87.3% were term or post-term; gestational age was unknown in 5.7%. The mean birth weight was 3270±453 g. Compared to in-hospital births, out-of-hospital
newborns had significantly higher rates of neonatal jaundice, macrosomia, hypoglycemia, sepsis, and polycythemia (p<0.001).
Conclusion: Challenges related to inadequate prenatal care and limited access to hospital services suggest that planned home birth is currently not a feasible option inIstanbul, Türkiye.
