Ev İlaçlaması Sonrası Alüminyum Fosfit Maruziyetine Bağlı 2 Kardeşin Ölümü

Abstract

Giriş: Pestisit zehirlenmeleri mesleki maruziyet, intihar, kazara maruz kalma gibi durumlarla ülkemiz ve dünyamız için büyük bir halk sağlığı sorunudur. Pestisitler, tarım endüstrisi yanı sıra ev ilaçlamasında haşere kontrolü için yaygın olarak kullanılmaktadır ve uygunsuz kullanımları hayati tehlike oluşturabilmektedir. Klorfenapir, binalarda haşere mücadelesinde kullanılan bir proinsektisittir. Maruziyet sonrası akut etkileri silik olup, latent bir dönemi takiben mitokondride oksidatif fosforilasyonu bozarak ölümcül hasara yol açabilir. Alüminyum fosfit ise tahıl depolarında kullanılan bir pestisittir; tablet formundadır ve nemle temas ettiğinde sitokrom c üzerinden hücresel solunumu inhibe eden “fosfin gazı” açığa çıkar. Amaç: Bu çalışmada yaklaşık bir hafta arayla sırasıyla etken maddesi “klorfenapir” ve “alüminyum fosfit” olan pestisitlerle evleri ilaçlandıktan sonra ölen iki kardeş olgunun sunulması amaçlanmıştır. Olgular: Her iki olgunun otopsilerinde küçüklerin ölümüne etkili travmatik bulgu saptanmamış olup, makroskobik ve mikroskobik olarak tüm zehirlenmelerde görülebilecek ödem, konjesyon ve hiperemi gibi nonspesifik bulgular saptanmıştır. Toksikolojik incelemelerde herhangi bir toksik madde saptanmamıştır. Olay yeri ve adli tahkikat bulguları ile birlikte küçüklerin ölüm nedeninin “alüminyum fosfit” zehirlenmesi olduğuna karar verilmiştir. Tartışma: Çalışmada sunulan olgulardan yola çıkılarak, ev ilaçlamasında bir hafta arayla kullanılan “klorfenapir” ve “alüminyum fosfit” in kullanım alanları, maruziyet sonrası akut ve kronik dönem etkileri, zehirlenme bulguları, insan vücudu üstündeki etki süreleri tartışılmış, pestisitlerin güvenli kullanımı için alınması gereken önlemlerin önemine vurgu yapılmıştır.

Two Siblings’ Death Following Home Fumigation With Aluminum Phosphide

Abstract

Introduction: Pesticide poisoning represents a significant public health concern both globally and in our country, arising from occupational exposure, intentional ingestion or accidental contact. Although pesticides are widely used in agriculture and domestic pest control, their improper use poses life-threatening risks. Chlorfenapyr is a pro-insecticide used for indoor pest control. Its acute effects are subtle; however, following a latent period, it can cause fatal damage by disrupting oxidative phosphorylation in mitochondria. Aluminum phosphide, a pesticide employed in grain storage facilities, is formulated as tablets. Upon exposure to moisture, it liberates “phosphine gas” that inhibits cellular respiration through a cytochrome c-mediated mechanism. Objective: The aim of this study is to present two sibling cases who died following the sequential domestic application of “chlorfenapyr” and “aluminum phosphide”, one week apart. Cases: Autopsy findings revealed no signs of trauma that could cause death. Macroscopic and microscopic examinations showed nonspecific findings, such as edema, congestion, and hyperemia. No toxic substances were detected in toxicological analyses. Combined with crime scene and forensic investigation data, the cause of death was determined as “aluminum phosphide” poisoning. Discussion: This study discusses the usage contexts of chlorfenapyr and aluminum phosphide when applied within the household one week apart, their acute and chronic effects following exposure, clinical manifestations of poisoning and their duration of action in the human body. Emphasis is placed on the importance of preventive measures to ensure the safe use of pesticides.
