Abstract

Osteomyelitis is an inflammatory condition of bone and bone marrow that develops in the jaws after a chronic odontogenic infection, trauma, tooth extraction or for a variety of other reasons. It is more common in the mandibula than the maxilla because of the dense, poorly vascularized cortical plates and the single blood supply from the inferior alveolar neurovascular bundle. The disease may be acute, subacute or chronic. Chronic osteomyelitis may become a refractory condition in the oral and maxillofacial region. Therefore, correct diagnosis of osteomyelitis is crucial. Therapy requires multidisciplinary approach. Chronic suppurative osteomyelitis can be treated by a combination of antimicrobial therapy and surgery consisting of sequestrectomy or decortication. Hyperbaric oxygen is often recommended as an adjunct in treatment of this disease. The aim of this report is to present a case of chronic suppurative osteomyelitis seen in a 21 years old male patient diagnosed by clinical, radiological and histological examination

Abstract

Osteomiyelit OM kronik odontojenik enfeksiyon, travma, diş çekimi v.b sonrası mikroorganizmaların çene kemiklerine inokülasyonu ile gelişen kemik ve kemik iliğinin iltihabi durumudur. Mandibulanın dens yapısı, kortikal kemiğin vaskülarizasyonunun maksillaya oranla az olması ile mandibulanın beslenmesini sadece alt alveoler nörovasküler yapıdan sağlaması mandibulada OM’in daha çok görülmesinin sebepleridir. OM; akut, subakut ya da kronik seyirli olabilir. Kronik OM oral ve maksillofasiyal bölgede inatçı seyreden bir tablo gösterebilir. Bu nedenle OM’in doğru teşhisi çok önemlidir.OM tedavisinde multidisipliner yaklaşım gerekmektedir. Özellikle uzun dönem seyreden inatçı OM’lerde antimikrobiyal tedavinin yanı sıra, sekestrotomi ve dekortikasyonu içeren cerrahi tedavi ile yardımcı tedavi olarak hiperbarik oksijen uygulaması düşünülmelidir.Bu vaka sunumunda 21 yaşındaki erkek kronik osteomiyelit hastasının klinik, radyografik ve histolojik incelemeleri ve değerlendirmelerine yer verilmektedir
