Recurrent Pyogenic Granuloma Gravidarum: Associated With Actinomycosis

Abstract

Pyogenic granuloma gravidarum is a pyogenicgranuloma which develops on the gingiva duringpregnancy. Actinomycosis is an infectious diseasethat is characterized by granulomatous and suppurative lesions caused by actinomycetes. The diagnosis of actinomycosis depends largely on the presenceof sulfur granules in the exudates. Here we report acase of pyogenic granuloma gravidarum associatedwith histopathologic findings of actinomycosis. A 24-year-old female patient with her first pregnancy was consulted to our clinic due to a left mandibular premolar mass that was spontaneously bleeding.Lesion was excised under local anesthesia. After sixweeks, the lesion was recurred with increasingbleeding. The recurrent lesion was treated with shortterm intra-lesional corticosteroid injection and subsequent excision. Although the lesion was not typicalfor actinomycosis, the histopathological examinationshowed findings of pyogenic granuloma gravidarumand associated typical sulfur granules. The first lesionwas treated with penicilline and the recurrent lesiondid not show any findings of actinomycosis. In this case, during bleeding, the granuloma gravidarum was thought to be infected with actinomycetesof oral flora. Since the lesion was removed in itsearly stage before the formation of pus, there was noclinical sign of actinomycosis

Aktinomikozis ile İlişkili Rekürrent Piyojenik Granüloma Gravidarum

Abstract

Piyojenik granüloma gravidarum hamilelik sırasındagingivada geliflen bir piyojenik granülomadır.Aktinomikozis ise aktinomiçeslerin neden olduğugranülomatoz ve supuratif lezyonlarla karakterizeinfektif bir hastalıktır. Aktinomikozisin tanısıçoğunlukla eksudada sülfür granüllerinin varlığınadayanır. Bu makalede aktinomikozisin histopatolojikbulgularıyla iliflkili bir rekürrent piyojenik granülomagravidarum olgusu sunulmufltur. 24 yaflında, ilk gebeliği olan kadın hasta sol alt premolar bölgede spontan kanamalı kitle dolayısıylakliniğimize sevk edildi. Lezyon lokal anestezi altındaeksize edildi. Altı hafta sonra artmıfl kanamaylarekürrens izlendi. Rekürrent lezyon kısa dönem intralezyonal kortikosteroid enjeksiyonunu takiben eksizyon ile tedavi edildi.Lezyon klinik olarak aktinomikozis için tipik olmamasına rağmen histopatolojik olarak tipik sülfür granüllerinin varlığıyla birlikte piyojenik granüloma gravidarum bulguları izlendi. İlk lezyon penisilinle tedaviedildi. Rekürrent lezyon aktinomikozis bulguları göstermedi.Bu olguda kanama sırasında granüloma gravidarumun oral floraya ait aktinomiçeslerle infekte olduğudüflünüldü. Lezyon, püy formasyonundan önce erkendönemde eksize edildiği için aktinomikozisin klinikbelirtileri yoktu
