Surgical extraction of third molar is the most common surgical procedure in Oral and Maxillofacial surgery. Despite of the adequate effort to perform these surgeries carefully, it may result in a number of complications. One of the rare complications after third molar surgery reported in the literature is bur fracture. This case report presents surgical management of broken and forgotton bur in the bone during third molar extraction surgery. Unintentional retained broken bur pieces was removed successfully by surgical operation. It is one of the main responsibility of the oral surgeon to check the surgical instrument for breakage signs and not leaving instrument fragments behind in the wound area.
