A case of Fasciola hepatica presenting with neuropathy

Abstract

Fascioliasis is a zoonotic disease caused by Fasciola species, especially Fasciola hepatica in humans. Fascioliasis is a commonly seen in herbivorous domestic animals such as sheep and cattle. Human infection usually takes place with the consumption of watercress. Adult Fasciola hepatica is a 2.5-3 cm long leaf-shaped flat worm 1.5 cm in diameter. When Fasciola hepatica cysts are taken orally they turn into larvae form in the duodenum. Larvae passes the wall of the small intestine through the peritoneum, and then penetrates the liver capsule and migrate in the liver parenchyma finally reaching the bile ducts. The migrating larvae in the liver parenchyma may cause damage by necrosis and fibrosis. Adult Fasciola hepatica may also cause thickening of the proximal biliary tree leading to partial obstruction of the bile ducts. Generally, the disease occurs with the signs and symptoms of infection and cholestasis. We herein present a case of Fasciola Hepatica emerging with neurological involvement, neuropathy.

Nöropati ile başvuran Fasciola hepatica olgusu

Abstract

Fascioliazis insanda fasciola türlerinin, özellikle Fasciola hepatica'nın neden olduğu zoonotik bir hastalıktır. Fascioliazis koyun, sığır gibi ot yiyen evcil hayvanlarda yaygın olarak görülmekle beraber insanlar rastlantısal konaklardır. İnsan enfeksiyonu genellikle koyun yetiştirilen yerlerde su teresi tüketilmesiyle alır. Erişkin Fasciola hepatica 2.53 cm boyunda 11.5 cm çapında yaprak şekilli bir yassı solucandır. Fasciola kistleri asıl konak veya insanlar tarafından oral yolla alındığı zaman duodenumda kist yapısından çıkarak larvalara dönüşürler. Larvalar ince barsak duvarını geçerek peritona geçer, daha sonra karaciğer kapsülünü penetre eder ve karaciğer parankiminde ilerleyerek safra kanallarına ulaşırlar. Göç eden larvalar karaciğer parankiminde nekroz ve fibrozis yoluyla hasara yol açar. Erişkin Fasciola hepatica'lar da safra kanallarında kısmi tıkanıklığa yol açarak proksimal safra ağacında kalınlaşma, dilatasyon ve fibrozise neden olabilirler. Genellikle enfeksiyon ve kolestaz belirti ve bulgularıyla hastalık ortaya çıkar. Biz burada nörolojik tutulum, nöropati ile ortaya çıkan bir Fasciola hepatica olgusu sunuyoruz.
